CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This Contract made and entered by and between:

DEPARTMENT OF EDUCATION (DEPED), CATICUGAN HIGH SCHOOL, with office address at

Progresso St., Poblacion III Siaton, Negros Oriental, herein represented by ESTERIA J. MACAJELOS,
Secondary School head, hereinafter referred to as the “First Party”;

-and-

MARLON M. ANNANG VERDOCILLO, of legal age, married, Filipino and with residence address at
Caticugan, Siaton, Negros Oriental, hereinafter referred to as the “Second Party’;

WITNESSETH

1. That the First Party is in need of the services of the Second Party to augment security
services during nighttime within school premises.

2. That the Second Party has signified his intention, to which the First Party has accepted,
to provide the service needed by the latter;

3. That the Second Party hereby attests that he is not related within the third degree of
consanguinity or affinity to the: 1) hiring authority and/or 2) representative of the First Party;
that he has not been previously dismissed from the government service by reason of an
administrative offense; that he has no derogatory records pending any police agency and the
courts; that he is not more than Sixty-five (65) years old;

4. That in view hereof, the Second Party is hereby contracted as School night security for
the period of 6 months from March 1 to September 30, 2020 in consideration of the monthly
honorarium of Two thousand pesos only (Php2,000.00) per month, to be paid on a monthly
basis;

5. That payment of an honorarium shall be based on representation of the DAILY TIME

RECORD, verified by the School Head of this Office;

6. That the Second Party is expected to be at the school premises 7:00 PM t0 5:00 AM to
ensure the security of the school most especially from burglars and thieves;

7. That the second party is expected to fill water containers of the school in times where
barangay water flow happens at night;

8. That the Second Party shall perform work at a time and schedule to be agreed upon by
both parties from 7:00 PM to 5:00 AM every day;

9. That any misrepresentation of information regarding the qualifications and credentials of

the Second Party discovered during the term of this Contract shall cause the immediate
termination of this Contract;

10. That it is understood that this Contract does not create an employer-employee relationship
between the First Party and the Second Party that the services rendered hereunder are not
considered and will not be accredited as government service; and that the latter is not
entitled to benefits enjoyed by the regular personnel of the First Party.
 IN WITNESS WHEREOF, both parties have hereunto set their hands this 1st day of July
2018 at Siaton, Negros Oriental
