INSTRUCTION SHEET For any queries, regarding any part of the admission process, you may ring up TISS C.A.R.E. (Centre for Applicant Relationship and Engagement): 022 - 4011 0457 Before registering, all candidates are expected to read the Information Brochure and Instruction Sheet carefully. Candidates registering first time are required to use the NEW REGISTRATION and for subsequent editing go to “Log in” using your registered email ID and the password. Kindly note that you will not be able to edit any of your details, once you click the “Final Submit” button in Step 5. You should use it only when you are sure that all information is correctly filled up including remittance details. You can keep editing the same using “Save and Exit” and NOT the Final Submit. The processing fee of Rs. 800/- per programe may be remitted by downloading the challan available in Step 5 into any branches of State Bank of India which is the only method of remittance available to candidates. The Institute will reject applications received in any other form of remittances. Candidates belonging to SC/ST whose parents’/guardian’s income for the last financial year, i.e. 2009-10, was less than Rs. One lakh, are exempted from payment of processing fee. However, they should attach the attested copy of Income and Caste Certificates along with their application. All candidates are required to preserve a copy of the print out of application form and the challan stamped by the Bank till you complete the selection process. When you communicate with the Institute, please quote your Application Serial Number, centre chosen, category etc. for easy and quick redressal of your grievances. Any question which is not relevant to you may please be answered “Not Applicable” instead of “No”. For example, “If you are a medical/para-medical graduate” may be answered “Not Applicable”, if you are not medical or para-medical graduate. Similarly, “have you passed post-graduate” may be answered “Not Applicable” instead of “No” OBC candidates who do not possess and also not able to submit the Non- Creamy Layer status certificate will be considered as general candidates Step 1: Personal Details Item 1 Name: Type your name, as recorded in the High School (Class X or XII) Certificate by your Board in CAPITAL LETTERS. Example: If your name is Vidya Vasant Mhatre: VIDYAVASANTMHATRE If your name has more than 32 characters, abbreviate it suitably to accommodate within the space provided. For example, the name Anand Swaminathan Vishwanathan can be abbreviated as. A N A N D S. V I S H W A N A T H A N Item 2 Gender: Click the appropriate button. Item 3 Date of Birth: Enter your date of birth as given in your High School (Class X or XII) Certificate in the space provided. Example: If your date of birth is 7th June, 1983, fill in as 070683 Item 5 Choice of Centre: Candidate should select the centre where he/she wishes to appear the Written Test. The Institute may cancel any test centre and/or may assign a nearby centre indicated by the candidate, if situation demands. Request for change in the centre for written Test will not be entertained. Item 6. Category: Select only ONE of the appropriate category that you belong to: (If a PWD candidate happens to be SC / ST candidate, he/she can choose either the SC/ST or PWD but not both) If you are a PWD, please select in the application form against the type of disability. If your religion is not listed out, you may choose “Others”. Item 7. Type the complete address for communication in BLOCK LETTERS. All the details including your FULL NAME, PIN CODE, MOBILE NUMBER, TELEPHONE NUMBER WITH STD CODE, to enable the Institute to make communication with you. Once you submit the application to the Institute, please do not change your E-mail ID and the Mobile Number, as important messages are going to be sent to your mobile & E-mail only. Item 8. Paste your latest stamp size photograph and also sign within the box in two places after taking the print out of your hard copy application form. Item 9. SC/ST/OBC (Non-Creamy)/Religious Minorities/PWD candidates can attend an orientation programme free of cost. If interested, you may choose “Yes” and also choose the nearest centre available in the list. Though it is not mandatory, candidates are encouraged to attend it. Step 2: Educational Details Item 1 Specify the pattern of study of your Bachelor’s Degree. If you are an engineering or medical graduate you should choose 10+2+4, graduates from Science, Commerce, and Management can choose 10+2+3, graduates from the VETERINARY Science, Pharma, etc., can choose any other pattern. Item 2 Specialisation or Major subjects : You are expected to write your major/Special/Specialisation of subjects that you studied in your Bachelor’s or Master’s Degree. For example: if you have done your graduation in Arts with Economics and Political Science, you may write B.A. in Economics and Political Science. If you have done your engineering in Computer Science, you may write B.E./B.Tech. in Computer Science and Networking. If you have passed your graduation in Physics and Mathematics, you may write B.Sc. in Physics and Mathematics. Item 3 Faculty: Science and Commerce graduate will choose “Science/Commerce”, Law/Arts/Engineering graduates should choose their respective subject. Item 4 You are required to write your marks/grade points/G.P.A. of your First, Second, Third and Fourth year, if any. If you have completed semester based graduation, you may add the marks scored in the first & Second semesters and put the average marks of two semesters and write the percentage also. Similarly, you do for the rest of the semesters and years. Five years integrated course candidates need to write the first three year marks; the fourth and fifth year marks can be neglected. Engineering candidates who took admission directly to second year need to apply their third year marks of diploma in the first year and second year marks in the second year column. You can enter your Grade Points in lieu of marks. If equivalent percentage is given, you may enter percentage of marks – if not available, percentage column can be ignored. (d) Provisional / Completion Certificate or Final Year mark sheet showing the class or consolidated mark sheet. The Candidate has already passed his/her Bachelor’s Degree examination on the date of applying for the programme(s). (e) Bonafide Certificate issued by the College indicating that the student is studying in the final year of the Bachelor’s Degree course in the current academic year, if pursuing final year.
