DEED OF DONATION

KNOW ALL MEN BY THESE PRESENTS:

This Deed of Donation is entered into on the ________________________ in

Marikina City, Philippines.

__________________________________, Filipino of legal age, married and a resident of

______________________________________________________________hereinafter
referred to as the DONOR;

In favor of

The BARANGAY CONCEPCION UNO, CITY GOVERNMENT OF MARIKINA, a local

government unit duly organized and existing under the laws of the Republic of the Philippines,
hereinafter referred to as BARANGAY CONCEPCION UNO, MARIKINA CITY with office
address at No. 1 A. Flores Street, Barangay Concepcion Uno, Marikina City, herein
represented by HON. GERARDO F. STO. DOMINGO, as the DONEE. Pursuant to
Sangguniang Barangay Resolution No.: ____ series of 2019; copy of which is attached.

WITNESSETH:

1. That the DONOR is the true and absolute owner of one (1) unit of FIRE
TRUCK more particularly described as follows:

One (1) Mitsubishi Fire Truck, 1994 Model, Diesel Fuel with
Plate No. RLR930, Serial/Chassis No. FP415J-550083,
Motor No. 8DC9-422262

2. That the DONOR is willing to donate the above-described motor vehicle to

BARANGAY CONCEPCION UNO, MARIKINA CITY.

3. That as an act of generosity with its generosity with its growing awareness
and appreciation of DONOR’S civic-mindedness and social commitment, and
coupled with its willingness to assist and support the meaningful programs
and projects for development and to further foster that camaraderie and
friendship, the DONOR hereby voluntarily GIVES, TRANSFERS AND
CONVEYS, by way of DONATION, unto the DONEE the above-described
motor vehicle with all the accessories found thereon, free from all liens and
encumbrances and full warranty of title and possession under the law.

4. That the DONEE hereby accepts and receives this donation made in its favor
by the DONOR and hereby expresses its appreciation and gratefulness for
the kindness and generosity of the DONOR.

5. That the DONEE undertakes the obligation to pay for the freight cost, and
expenses for conversion of the fire truck into a Left Hand Drive vehicle.
