Organization Attestation

Definitions
“Organization” refers to the organization that will be signing the funding agreement. In the case of a proposal submitted by a
consortium, where multiple organizations will sign the funding agreement, every organization must submit a signed
Organization Attestation.

Instructions
This attestation must be dated and signed by the Organization’s Chief Financial Officer, or a duly authorised board member.

This attestation is subject to validation by Global Affairs Canada.

Regarding the project proposal above, I attest that the organization meets the following criteria:

1. Eligible costs

The Organization has read and based the Project budget on eligible costs outlined in the "Guidance on Eligible Costs for
Development Initiatives" document.

2. Cost share

In accordance with the Global Affairs Canada "Policy on Cost-Sharing for Grant and Non-Repayable Contribution
Agreements", the Organization must provide a minimum of 5% of the total eligible direct project costs over the life of the
Project in cash and/or in kind. (In-kind contributions refer to materials, goods or services to which a dollar value can be
attributed that would otherwise be purchased and paid for by the Organization to achieve the project results. All cost share,
including in-kind contributions, must be documented and auditable.)

The total eligible direct project cost refers to the sum of the contribution of both the Government of Canada’s contribution and
the Organization’s contribution to direct eligible costs. Please refer to the "Guidance on Eligible Costs for Development
Initiatives" to help calculate the total direct eligible costs for the Project. Note that overhead is not a direct eligible cost.

If there will be multiple signatories to the funding agreement, not all signatories are required to contribute to cost share but all
are jointly and severally accountable for ensuring that the cost share commitment is met. Should cost share be provided by
more than one signatory, all signatories’ contributions should be counted as part of the total cost share requirements. Each
signatory must attest to the full cost share amount for which it is jointly and severally accountable in the space below.

NOTE: This is not mandatory if the Organization has read the "Policy on Cost-Sharing for Grant and Non-Repayable
Contribution Agreements" and the Policy is not applicable to the Project.

The Organization will commit to ensuring a minimum of % of the total eligible direct project costs are provided through
cost share over the life of the Project.

3. Financial capacity

The Organization has sufficient financial capacity to undertake the Project without creating any adverse impact on its ability to
deliver and meet its financial obligations and commitments under existing agreements funded by the Department.

4. Sexual exploitation and abuse - Code of conduct

Recognizing that sexual exploitation and sexual abuse violate universally recognized international legal norms and standards
and, aligned with Canada’s commitment to the delivery of international assistance grounded in a human rights framework, the
Organization declares and guarantees that it has, or will have by September 1st, 2019, a publicly available code of conduct
(CoC) to prevent, investigate and respond to sexual exploitation and sexual abuse. The CoC expectations and applicable
definitions can be found in Section 22.6 of the "Contribution Agreement – General Terms and Conditions for the International
Development Assistance Program".

5. International sanctions

The Organization declares and guarantees that the funding for the purposes of the Project will not knowingly be used, either
directly or indirectly, to do business with countries or persons subject to sanctions imposed by Canada or the United Nations
under the Special Economic Measures Act, S.C. 1985, c.17 the United Nations Act, S.C. 1985, c. U-2, the Export and Import
Permits Act, S.C. 1985, c. E-19, the Justice for Victims of Corrupt Officials Act, S.C. 2017, c.21, and all other international
conventions related to sanctions to which Canada has adhered. A general list of, and information about, sanctions currently in

6. Anti-terrorism

The Organization hereby declares and guarantees that the funding for the purposes of the Project will not knowingly be used
either directly or indirectly to benefit terrorist groups, or individual members of those groups, or for terrorist activities, as defined
in the Anti-Terrorism Act, S.C. 2001, c. 41 and the Criminal Code, R.S.C., 1985, c. C-46. The Government of Canada list of
