b. 2x Photographs duly attested by HOD/stamped
c. Photo copy of Registration Book of the Vehicle. If the vehicle is not registered then
an affidavit worth Rs. 20/- will be provided by the applicant specimen attached.
iii. Submit the complete form to the security office through proper channel.
iv. Incomplete Application form will not be entertained.
