1.0 Introduction
FilHigh Builders Trading and General Services Corp - Contractor. is fully aware of its
obligation to implement its safety management throughout the construction period in
accordance with the safety standards in force, all the applicable laws and regulations
of the Republic of the Philippines and in accordance with the Contract, in order to
safeguard all the persons and properties from the effects of any actions undertaken
by Filhigh in the carrying out and completing the Works. This Safety Program has
been prepared to ensure that Filhigh will manage the Site with a determined view of
“Safety First” from the commencement until the Substantial Completion of the Works.
2.0 Policy
This Safety Program has been prepared based on the policies as mentioned below:
a. To establish the Safety Program through implementation of which the safe and healthy
working environment will be secured for the staff and workers who will be engaged in
the construction activities, for the staff and members of the PMO and the Engineer, and
for the third parties who will be affected by the construction activities;
b. To prepare the Program incorporating all the measures in a concrete way to ensure
that such measures will be physically implemented effectively and efficiently; and
c. To make the Program a common recognition of all those engaged in the construction
activities to ensure that the daily activities will be undertaken in a safe manner.
d. To submit the Program, upon approval and endorsement for implementation by the
Department Of Labor and Employment’s Bureau of Working Conditions, to the Engineer
for evaluation and review on the Program’s compliance with the specifications called for
by the Bid Documents.
3.0 Objective
FilHigh Builders Trading and General Services Corp Plan has been prepared with an
objective of creating a safe and healthy working environment by implementing the
Safety Program carefully prepared based on the policies mentioned immediat ely
above.
4.0 Scope
This Safety Program will be applicable to the areas identified as the Site in the
Contract Drawings, the areas borrowed by Filhigh outside the Site (such as quarries,
borrow pits, accommodation for workers, subcontractors’ off-site compound areas, etc.),

the areas and the traffic which may be affected by drains, dusts, noise, flash, vibration,
etc. as the direct results of construction activities.
For the areas mentioned above, including persons residing therein and properties
belonging thereto, FilHigh will comply with the relevant laws and regulations in terms
of safety and health, and will comply with this Safety Program as well.
5.0 References
Laws and regulations prevailing in the Philippines, to, which the Contractor must adhere
to.
The Joint Venture remains committed to strictly comply with the local labor and safety
regulations currently enforceable in the Philippines – specifically the DepartmentOf Labor
and Employment’s Department Order No. 13 on the “Guidelines Governing Occupational
Safety and Health in the Construction Industry”, and Rule 1410 of the Department Of
Labor and Employment’s “Occupational Safety and Health Standards on Construction
Safety”.
6.0 Responsibilities
6.1 Roles and Authorities of Members of Safety Organization
6.1.a Project Manager
The Project Manager shall be fully responsible for safety matters in relation with the
Works to ensure that all works engaged on work site are implemented in a manner
pursuant to the relevant laws and regulations promulgated by the Department Of Labor
and Employment for the construction industry, as well as relevant contractual
requirements on safety and health affairs.

6.1.b Safety / Security Manager
The Safety / Security Manager shall, directly under the control of the Project Manager,
assist various departments by advising, inspecting and formulating safe operating
procedures and practices.
He shall:
1. Conduct regular site inspections jointly with all subcontractors and the departments
concerned, to ensure safe working conditions and methods, adequacy of life support,
fire fighting, including precautionary measures to prevent them, safety equipment and
to detect potentially hazardous condition.
4. Conduct safety orientation and training courses, lectures etc. for all personnel.
6. Ensure that the all safety personnel of both the FIL HIGH and those of the
subcontractors understand potential danger of chemical and toxic materials and
familiarize themselves with the precaution required to safeguard their health.
9. Promptly comply with the required report submissions to FIL HIGH management,
to the Engineer, and to the Department of Labor and Employment (Bureau of Working
Conditions) on all affairs related to the health and safety.
10. Maintain good housekeeping within the entire project site and other vicinities
affected by the project’s construction activities, to ensure that construction debris, all
forms of scrap, waste oil, chemical and other refuse are properly disposed of in
accordance with the procedures and destinations agreed upon with the local office of
the Department of Environment and Natural Resources.
11. Ensure that not only the safety staff but also all personnel at the jobsite are able
to effectively operate fire fighting, safety equipment and familiarize themselves with
emergency evacuation procedure and layout.
Under the direction of the Safety / Security Manager, the Managers shall:
1. Ensure that there is a safety system for all the works executed under their control.
2. Provide adequate facilities tools and equipment to ensure that all work can be
carried out safely. Ensure that correct protective clothing and equipment are
provided and worn by the workmen to protect them from incidental injury and
safeguard health.
3. Ensure that effective corrective measures are taken to eliminate hazardous
conditions and practices.
4. Formulate civil and building department safe operating procedures and practices
and ensure their compliance.
5. Under the direction of the Safety / Security Manager, ensure the competence of
staff in performing their respective duty.
6. Ensure that their staff and sub-contractors’ supervisors and workmen attend
relevant safety trainings.
7. Ensure compliance with all safety procedures, guidelines and relevant safety
practices.
8. Ensure that accidents are adequately investigated, comprehensive required
reports are rendered, and that effective measures are taken to prevent recurrence.
9. Ensure that staff, and sub-contractor’s personnel, are kept informed of known
potential hazards that might affect the environment and ensure their awareness of
next course of action to minimize the risk and maintain the conservation of the
environment.
6.1.d Safety Officer Responsibility
The number of safety officers to be assigned at the jobsite must comply with the
provisions of the Department Of labor and Employment’s Occupational Safety and
Health Standards for high-risk activities, such as in the construction industry. He will
assist the Safety / Security Manager on all of his responsibilities and be a representative
of the Safety / Security Manager.
He is also responsible to directly supervise and monitor the activities and required
continuous presence of each subcontractor’s safetymen in monitoring their respective
scope of activities at the jobsite.
The Contractor recognizes that having made the places of work and methods
safe,management is still faced with dangers created by workers. In order to meet the
objective of making significant inroads into accident prevention, it is essential to make
both the subcontractors’ safetymen and workers aware of their responsibilities for safety
through education and training.
7.2.a FIL HIGH shall make sure that all new workers must go through safety induction
before they are allowed to work on site. Among the topics to be presented are:
1. basic rights and duties of workers at the construction site
2. means of access/egress both during normal work and in emergency situations
3. measures of housekeeping
4. location and proper use of welfare amenities and first aid facilities
5. proper care and use of the items or personal protective
6. Equipment and protective clothing provided the workers
7. site safety rules and regulations
8. traffic rules and precaution
9. unsafe practices and unsafe conditions
10. schedule of peso penalties for violation of site safety
11. rules and regulations
7.2.b As mandated by the Department of Labor and employment, specialized
instruction and training should be given the following groups of workers, to be
undertaken in-house by and duly certified by a D.O.L.E. accredited organization:
3. workers engaged in excavations of at least one meter deep or deep enough to cause
danger, shafts, earthworks, underground works or tunnels;
7. workers engaged in the erection of pre-fabricated parts of steel structural frames and
tall chimneys, and in concrete work, form work and other such work;
9. workers as signalers;
Training of workers by the immediate supervisor and foremen will be carried out
continuously – through the mandatory Daily Tool Box meetings. A record of training
attendees shall be maintained and compiled by the Safety Department, copies of which
must be provided the office of the Engineer.
7.3 Drills

In line with FilHigh’s commitment to continually maintain safe and healthy working e
nvironment within the entire project site during the period of its construction up to t he
turnover date, definite schedule of meetings shall be held to discuss, review and r ecommend
on how to further improve the safe and healthy condition at the work ar ea and its
immediate surroundings.
All such meetings shall be documented and kept on orderly files, copies of which sh all
be provided the Engineer’s office, and on periodic basis, to concerned bureau of t he
Department of Labor and Employment as mandated by law.
The meeting shall be held once in every month preferably on the first week of every
month, to discuss matters pertaining to safety, health, welfare and environmental prot
ection.
As this Committee is tasked with the responsibility of undertaking the planning and
policymaking group in all matters pertaining to safety, health and environmental prot
ection, its principal duties are:
3. directs the accident prevention efforts of the project in accordance with the safety
programs safety performance and government regulations in order to prevent acciden ts
from occurring in the workplace
6. submits reports to the management and the Engineer on its meeting and activities
9. Develops and maintains a disaster contingency plan and organizes such emergency
The meeting shall be minuted and it shall be distributed soon after the meeting to a ll
participants, FilHigh’s management, the Engineer and D.O.L.E.’s Bureau of Working C onditions.
The Safety / Security Manager shall prepare the monthly safety subject and agenda a s
a basis for the committee’s discussion.
Chaired by the Safety Manager, this weekly meeting provides the venue for the revie w
and evaluation on the on-site implementation of the project’s comprehensive safety program.
The subject matter scheduled for discussions during the meeting are as follow:
5. review and discuss needed actions for observations noted by the team’s weekly sit e
safety patrol
7. discussions on how to resolve major safety discrepancies noted for the week
Attendance for the weekly meeting is mandatory for all safety officers and all subcon
tractors’ safety men, absence from which is subject to the peso penalty regulation.
All site engineers/supervisors of Donggwang Clark Corp. and subcontractors must atte
nd the meeting, absence from which is subject to peso penalty regulation. Attendanc e
shall be documented and kept on file for future references.
Safety, health and environment shall be at the top among the agenda to be discusse d.
When an incident/accident has occurred at the jobsite, the concerned safety offic er
/safetyman of the involved worker must attend the meeting to explain the why, wh en
and how the incident happened, and what immediate course of action is being ta ke
n to prevent its repetition.
Where safety, health and environmental matters involving policies etc. are not able re
solved, the matter shall be forwarded to the Project Manager for decision.
Toolbox meeting shall be held daily each morning with team Supervisor/Foremen coo
rdinating the meeting prior to deployment to the worksite.
The meeting shall be made as brief but to the point as possible, preferably not exce
eding 15 minutes. Attendance shall be properly documented as evidence of complian ce
to this all-important pre-work operations’ safety procedure.
10.0 Reports
10.1 Safety Performance Report
One copy of the comprehensive monthly safety report will be distributed to the:
- Engineers
- Project Manager
- Site Managers
- Sub-Contractors
- Dates, time and places where accidents happened (including fire, traffic accidents, d
amage by windstorm, electric shock, etc.)
- Dates, time and places of Safety Meetings held (including attendees, matters discus
sed and decided, etc.)
- Dates, time and places of Safety inspections conducted (including attendees, subject s
of inspection, findings, actions taken, etc.)
Attachment “C” is the Accident Report Flow Chart – showing the flow of information
(verbally and formal report) to all concerned parties on any incident/accident occurrin g
at the jobsite.
1. Any Incident which has the potential to cause personnel injury or damage to prop
erty, either to project owner, FilHigh’s or Sub contractor’s manpower or equipment.
2. Any personnel injury, even those which requiring only First-Aid Treatment.
4. Any incident involving a private vehicle used for official business for the project w
here injuries were sustained as a result of that incident.
Any of the above incidents occurring within the project site must immediately be rep
orted even orally to the Safety / Security Manager, with the formal incident report fo
llowing soon after.
Every incident involving any vehicle or equipment on official use for the project must
immediately be made known to the Safety / Security Manager. This procedure is very
important so that all concerned parties could be accordingly informed, and required
actions/coordination maybe taken promptly, as maybe dictated by the situation.

The driver concerned must complete the formal incident report as soon as feasible a
nd submit this to the Safety / Security Manager.
Likewise, the driver is responsible to report to the police within 24 hours if the accid
ent involved injuries or damages to any person, or properties.
The supervisor of the injured person must report the incident to Safety / Security Ma
nager or Officer, initially an immediate oral report, then later, by accomplishing the re
quired Accident Report forms.
- Site Manager

b. Provide communication link in ensuring that the top management are kept consta
ntly informed of the company’s safety performance.
It is very important that all accidents are reported to the company’s top managemen t
and the Engineer at the shortest possible time. This is to enable the company to: a. Attend
immediately to the accident or its victim and render all necessary assistanc e. b. Investigate and
recommend necessary preventive or remedial measures to preve nt reoccurrence of similar
accidents. c. Notify the insurance company and the concern ed government agencies, as the local
police and D.O.L.E.
d. Dangerous Occurrences
e. Any other accidents or unusual events that took place in the course of work.
For all occurrences of accidents, the supervisor, foreman, or any persons directly in-ch
arge of the job shall be responsible to report the accident without any delay to the
Safety Manager or Project Manager who, in turn, shall render the initial oral report t o
the Engineer and the regional office of D.O.L.E. having jurisdiction of the project sit e.
The concerned supervisor/foreman, upon being informed of the accident, shall promp tly
prepare and submit the required formal report to the Safety Manager, using the s tandard
Donggwang Construction Report Form.
The responsible person is required to secure and submit a Police Blotter Report – ess
ential for claims from insurance and other parties involved.
Special attention should be given in preparing the written report. The Project Manag er,
Supervisors, and the worker concerned and the Safety Officer should discuss th e
cases and the necessary preventive measures to prevent possible reoccur rence.
A copy of the accident report should be circulated among the concerned managerial
and supervisory staff and the members of the Safety and Health Committee to rend er
them knowledgeable on the causes and the specific preventive measures taken in eliminating
the recurrence of similar accident.
In line with this objective, there are three levels of site inspections to be regularly un
dertaken during the entire period of the project:
11.1 Daily Safety Patrol – to be undertaken by FilHigh’s safety officers and indi
vidual subcontractor’s safetyman/officer to monitor and insure full compliance of all w
orkers to safety and health rules and regulations, and the maintenance of safe workin g
condition. On-thespot apprehension of violators and/or rectification of unsafe condi tions or
work stoppage order are undertaken, properly documented with the appropri ate report form
‘Unsafe Work Report”, attached as Attachment “D”.
11.2 Weekly Site Safety Patrol – To be undertaken by all safety officers and s
ubcontractors’ safetymen/officers, headed by the Safety Manager, to audit the overall
site condition and observe compliance of workers on the stipulated site safety rules a
nd regulations. Apprehension of violators, rectification or work stoppage for unsafe w
orking condition is undertaken as needed.
Observations of each individual safety staff is recorded in the prescribed form, read a
loud before the group on specific deficiency, name of violator (if any), particular sub
contractor involved. On-site brief discussion is made followed by group decision on I
mmediate remedial action to be taken. Major safety discrepancies noted during the p
atrol are also discussed during the weekly safety meeting, both to call the attention of
the safetymen concerned, and to avoid recurrence of such situations at the jobsite.
Attendance for all safetymen/officers is mandatory, subject to peso penalties for unac
ceptable reason for absence.
Before any hazardous job may be performed on the worksite, FilHigh will hold a Pre-
Job-Planning session with the Sub-Contractors concerned.
An inspection of the work-site and the equipment involved shall be conducted before
the commencement of the dangerous work.
All dangerous activities shall be controlled and properly documented via formal writte n
work permit approved b the Safety / Security Manager.
This work permit is required whenever specific task will/may cause risk to life or dam
age to property such as:
All workers shall wear suitable safety equipment when working in dangerous areas de
signated by the Joint Venture, except areas specifically designated as non-protection a
rea.
- Workers who are working from unguarded surfaces six (6) meters or more above g
round, temporary or permanent floor platform, scaffold or where they are exposed to
the possibility of falls hazardous to life or limb, must be provided with safety harness
and life lines.
- Safety eye-goggles and gloves are mandatory to workers undertaking grinding and

strategic points within the worksite.
- Any worker caught failing to wear appropriate safety devices will be subjected to st
rong reprimand and the corresponding peso penalties. Proper safety procedures for t he
use of grinding, cutting and fastening equipments, isolating and controlling mecha nical
equipment and electrical circuits shall be observed strictly at all times.
- Safety helmet shall be worn at all times, except in areas specifically designated as
non-head protection area.
An emergency and first aid organization plan is to be set up at the working area, co
vering all personnel and equipment, means of communication and means of ways of
transportation, in compliance with the provisions of the Department of Labor and Em
ployment for hazardous workplace, contained under the Rule 1960 of the Occupation al
Safety and Health Standards, and Section 8 of the Department Order No. 13 on ‘G uidelines
Governing Occupational Safety and Health in the Construction Industry”.
Every worker will be informed of the plan, especially where the first aid posts are loc
ated.
Supervisor and/or responsible personnel will be made aware of the existing plan and
procedure.
All injuries should be reported, treated and recorded as soon as practicable at the ne
arest first aid post.
First-aid kit / box shall be provided at all guard posts within the jobsite, and every s
afetyman/safety officer is mandated to carry the kit at all times while within the peri
meters of the jobsite.
The kit or boxes will contain at least compress and triangular bandages, sterile gauge,
antiseptic, adhesive and forceps, a tourniquets, blunt-end scissors and splint.
All guard house/post and roving guards, and all safety personnel, both of Donggwan g
Construction and subcontractors, shall be required to carry first aid kits. Contents a nd
condition of the kit shall be submitted for inspection by the site emergency clinic on a twice
a month basis.
The doctor or nurse assigned at the emergency site clinic shall be responsible to full y
orient the safety staff and concerned security guards on the proper usage of the c ontents of
the kit.
14.3 Stretcher
Stretcher will be readily available as and when required to ferry any injured personnel
comfortably to safety for further medical attention. A blanket will be provided for eac h
stretcher.
A stand by vehicle suitable for carrying injured person to the nearest hospital will be
made available.
First-Aid Room for treatment of the injured shall be suitably located in the General C
ontractor’s office area.
The First-Aid Room will be equipped with medicines, medical supplies and facilities.
Location Plan of First Aid Kits and Emergency Treatment Room will be periodically an
nounced during the daily morning’s general assembly and exercises for all workers.
Basic medical facilities shall be installed at the emergency site clinic, to be located at
the Joint Venture office area. A full-time registered nurse who shall administer to min or
wounds and injury occurring at the jobsite including first aid shall initially man th e medical
clinic.
For major injuries and wounds, patients shall be rushed, on stretcher and to be tra
nsported by the standby vehicle, to the nearest hospital.
Written emergency procedures will be posted at suitable locations and shall be displa
yed in the Philippine and English languages as a matter of continuous reminders to
everyone at the jobsite. These procedures are among the major subjects discussed du
ring the required full orientation of new workers before they enter and work at the p
roject’s jobsite.
Access and escape routes are to be identified and shall be revised to suit whilst cons
truction is in progress. This shall among the reminders and updates to be mentioned
during the daily morning’s general workers’ assembly and at the Daily Tool Box meeti
ngs the stairwell shall have provisions for battery backup emergency lighting.
Fire extinguishers and other firefighting equipment will be properly located all over th e
work site with proper signage indicating such equipments for easy recognition.
shall be protected by barriers at all times and floodlight at night. Warning and di version
signs concerning roadwork shall be suitably placed to give motorist ample wa rning. During the
movement of heavy vehicles across roads or onto roads, men beari ng red flags shall be in
attendance to warn other road users and to generally control traffic in a safe manner.
16.1 Guardrails
Any places or working platform where a person is liable to fall from a height of 2 m
eters shall be provided with sufficient guardrails and toe boards and shall be properl y
constructed and:
- the distance between any toe boards or barrier and the lowest guardrails shall not
exceed 700mm.
Guardrail and toe board must be free from sharp edges and maintained in good rep air
to prevent hand injury or anything alike.
16.2 Platform
Working platform should be constructed with sound materials and the use of unsuita
ble material is prohibited.
No working platform should be used for any purpose until its construction is comple
ted and the necessary safeguards properly fixed.
Every working platform more than 2000mm above the ground or floor should be clo
sely boarded or planked.
Workers on the platform must be equipped with safety belt connected to lifelines wh
ere practical.
16.3 Barricade
Any working area or work that may pose a danger to other workers in adjacent area
must be temporally fenced off by means of warning tape, cones or temporary wall.
Appropriate signage must be posted to inform of the possible danger created by suc h
work.
The Contractor recognizes that many of the operations on the project will involve wo rk
at height and that falls from height regularly causes a significant number of seriou s injuries in
the Construction Industry. FilHigh and Subcontractor will therefore ensure that all such work is
properly controlled and that all such work has safe access and working platforms in
accordance with the local rules and regulations. Work at height will normally be
accomplished by the provision of scaffold work platforms of appropri ate dimension, fitted with
guardrails.
The Contractor ensures that all scaffolding is erected, altered or dismantled by compe
tent, experienced personnel under the immediate supervision of a competent supervis or,
and that the following requirements are observed:
1) All scaffolds will be of good construction, strong and sound materials, free from d
efect and properly maintained.
2) Standing scaffolds will be erected on firm, stable bases and be adequately strutted
and braced.
3) No scaffolds will be overloaded and, where necessary to prevent danger from falli
ng objects, will be fitted with protective screens or fans.
4) The Contractor ensures that all scaffolds are inspected by a competent person in
accordance with the requirements of the regulations and the results of the inspection s
recorded.
Where the provision of scaffolding for work at height is impractical, other safe workin g
methods or system will be used. This may be in the form of mobile elevating wor k platforms,
personnel carriers suspended from cranes, or the provision of all arrest e quipment. Where
such alternative methods of access/protection are used, a system of work will be devised to
ensure that all aspects satisfy the relevant statutory requirem ents, and provide the personnel
involved with a safe place and system of work.

The Joint Venture shall undertake whatever is practicable in its capacity to provide ap
propriate signage, which will be located at all ideally strategic locations.
Specifically, safety signage shall include, but not limited to the following high risk co
nditions at the construction site:
e. All places where contact with or proximity to electrical/facility equipment can cause
danger
f. All places where workers may get in contact with dangerous moving parts of machi
neries or equipment
All safety personnel and the roving security guards must continually monitor the prop er
placement and condition of all safety signage at the jobsite. Those seen damaged or missing
must be immediately replaced
Heavy peso penalties and Out-of-Jobsite rule must be strictly implemented for worker s

tified in accordance with the standard trade test prescribed by Technical Education an d
Skills Development Authority (TESDA) in coordination with its accredited organizatio n;
2. Likewise, all heavy equipment must be tested and certified in accordance with the
standards set forth by D.O.L.E. or its recognized organization – prior to commissionin g
of said equipment.
For convenience and practical purposes, the services of D.OL.E. have accredited organi
zation, First Phil. Skills & Equipment Testing Corp. shall be availed of for equipment
testing and operators training at the project site. The services of same D.O.L.E. accred
ited organization shall likewise be availed of, while at the jobsite, to undertake requir
ed training of workers doing critical work activities – in compliance with the requirem
ents of Section 15 of D.O.L.E.’’s Department Order No. 13.
1. Every lifting appliance including working gear and all other plant equipment used
for anchoring or fixing shall:
1. Every crane, crab and winch shall be provided with a brake to prevent the fall of t he
load and to control operation when the load is lowered.

3. The driver of every power driven lifting appliance shall be provided with a cabin
which shall:

Every chain or rope that terminates at the winding drum of a lifting appliance shall
be properly secured thereto and at least two turns or such chain or rope shall remai n
on the drum in every operating maximum end position of the appliance.
1. A lifting appliance shall not be operated other than by a person trained, competent,
physically fit, and authorized to operate the appliance.
2. When the operator of a lifting appliance has no clear and unrestricted view of the
load for safe working, there shall be appointed one or more signal men to give the
necessary signals to the operator.
3. Every signal given for the movement or stopping of a lifting appliance shall be dis
tinctive in character and the person to whom it is given is able to hear or see it eas ily.
1. Safe working loads shall be plainly marked on every lifting appliance and in case of
a crane with variable operating radius, safe load at various radii of the jib shall be marked on
the jib displayed in the driver’s cabin or fitted with an automatic safe loa d indicator.
2. In every derricking jib, the maximum radius at which the jib may be worked shall
be plainly marked on it.
1. No lifting appliance shall be used unless it has been tested and examined thoroug
hly initially and every year thereafter by a competent person by way of his training a
nd experience in such work.
2. No lifting appliance, which has undergone substantial alteration or repair affecting its
strength or stability, shall be used unless it is tested and thoroughly examined by a
competent person.
17.2.i Hoist
Every hoist way shall be efficiently protected by enclosures and when access to the h
oist is necessary, it shall be fitted with gates. Such enclosures and gates shall extend to
2.16 m. (6’6”) except when lesser height is sufficient to prevent the fall of persons and where
there is no risk of any worker coming in contact with any moving part of the hoist, but shall
in no case be less than 2.16 m. (6’6”).
The safe working load or the number of persons that can be carried in a hoist shall
be plainly marked on every platform or cage of the hoist. A notice prohibiting overl
oading of the hoist shall be placed on the platform or cage of the hoist.
1. Hoist for the carriage of goods and materials shall be of such construction that it is
operated outside of the cage unless the doors of the cage and the enclosure are of the
interlocked type.
2. Hoist for the carriage of persons shall have the doors of the cage and enclosure of
hoist way of interlocked type and the cage completely covered and fitted with ove rrun
devices.
No person shall be raised, lowered or carried by a power driven lifting appliance exc
ept:
3. when the use of hoist or suspended scaffold is not reasonable, provided that:
b. the winch used is so constructed that when control lever or switch is not held in
operating position, brake is applied and disengages from pawl and ratchet gears; and
i. a chair or cage, or
ii. a safe skip or other receptacle at least 1m. (3 ft.) deep, and
iii. measures are taken to prevent the chair, cage, skip or receptacle from spi
nning or tipping in a manner dangerous to any occupant.
1. After erection or alteration, every hoist shall be tested and examined every six (6)
months by a competent person and the result of such tests and examination shall be
recorded in a logbook maintained for the purpose.
2. The logbook shall be made available for inspection by the enforcing authority.
3. No chain, rope of lifting gear shall be loaded beyond its safe working load except
for the purpose of testing.
4. No chain, ring, hook, link, clamp, shackle, swivel or eyebolt altered or repaired by
welding shall be used unless it is tested and examined and its working load specified in
the test.
5. Hooks for raising or lowering of load shall have devices to prevent displacement o f
sling or load.
6. No double or multiple slings shall be used if the upper ends are not connected b y
means of shackle, ring or link of adequate strength or the safe working load is exc eeded.
7. Chains with knots or chains shortened by means of bolts and knots inserted throu
gh the links or by welding shall not be used.
2. When any equipment or part thereof is being dismantled, or repaired and a worke r
maybe endangered by the collapse or movement of the equipment, blocking shall be
installed to prevent collapse or movement.
4. A friction type clamp used in hoisting materials shall be constructed in a manner that

accidental slacking of the hoisting cable will not release the clamp.
5. When the operator of a shovel or similar machine is obstructed in the view of thepath
of travel of any part of the shovel or similar machine, one or more signal menshall assist
the operator by: a. keeping that part of the shovel or similar machine under observation
when it is out of view of the operator, and b. communicating with the operator using
prearranged signals or where these signals are impracticable, by audible communication
system.
a. have its rated capacity legibly cast or stamped in plain view on the jack, and
b. be equipped with a positive stop to prevent over travel or where a positive stop
Is impracticable, it shall be provided with a device indicating maximum allowable
travel.
10. Piles or sheet piling shall be adequately supported at all times during hoisting, p
hasing, removal or withdrawal and no worker who is not directly engaged in such op
eration shall be in the area where the operation are being carried out.
19.3 Operator The operator must not be handicapped in any way and he must
be competent to operate the particular equipment. He must be able to:
- know and comply with standard lifting hand signal (in the case of a crane).
The safe working load, as specified in the manufacturer’s instruction should not be ex
ceeded at any time.
The weight of load must be taken smoothly. Snatching can cause boom or rope failu re
where the weight of the load is close to the operating maximum, the load shall b e raised
several centimeters to check the stability of the crane.
Swinging shall be done slowly. Swinging too fast can throw the load out or radius a
nd cause overloading and the risk of striking adjacent objects. When a crane has a s
winging motion, a clear space shall be maintained between the crane body, the coun
terweight, and any moving parts of the crane and fixed object nearby to prevent per
sons being trapped and crushed when the crane swings.
No mobile crane shall be allowed to enter worksite prior to producing relevant certifi
cation and permit.
The hoisting mechanism of the crane must not be used for any purpose other than l
ifting a load vertically.
All chains ropes and lifting tackle must be of adequate strength constructed of soun d
No bolts or other material shall be placed between links of chain to shorten or splic e
it.
Slings shall be protected from damage due to sharp edges by wrapping edges with a
softer packing material.
Sharp bend shall be avoided in slings and they shall be protected from abrasion.
Eyebolts shall always be screwed up to collar; otherwise a sideways pull will stress th e
shank. If necessary, a shim shall be used.
The Safety / Security Manager will ensure that all slings are examined at least once
in every six months and that they are lubricated and stored in storage rack/area ab ove
ground level. Slings are to be examined for:
- Bird caging
- Bulges
- Broken Strand
- Crush damage (flattened strands)
- Core protrusion/damage
- Damage spliced
- Fatigue
- Gap or excessive clearance between strands
- Heat damage/electric and strikes
- Kinks or bends
- Rope stretch
Any sling found to be unserviceable due to defects (described above) is to be destro
yed by cutting and deleted from the sling record.
Chains, shackles, etc. shall be examined at least once in every six months for:
It should be clearly understood that the temporary nature of these supplies does not
permit a reduction in, or deviations from, safety standards for equipment/personnel p
rotection, equipment selection or electrical system design.
All temporary electrical installations and works on main portion will be operated and
checked by certified electrical workers. Daily inspections will be conducted on all inst
alled wirings and facilities.
On small temporary installations and systems using plugs, extension cables etc. the u
ser can create an arrangement to suit his needs without necessarily requiring an elect
rically trained person, but inspected regularly by a certified electrician.
A further significant factor to be considered is the very aggressive and abrasive envir
onment in which these plugs sockets, leads, tools, lights etc. are expected to operate.
This can lead to equipment becoming potentially unsafe over a very short period of use.
The implication of the previous comments is that user of such installations must visu
ally inspect such temporary equipment each and every time it is put into use and at
regular periods thereafter.
The user must ensure that no equipment is used or remains in service when its insp
ection date has been exceeded and no re-inspection has been completed.
21.0 Earthworks
21.1 Shoring and Timbering
1. The walls of every excavation over 1 m. (3 ft.) deep shall be supported by adequa te
shoring and timbering to prevent collapse, provided that this shall not apply to an
excavation.
c. the walls are sloped to forty-five degree (45) angle from the vertical or cut to the
angle of repose.
d. in which a worker is engaged in timbering or other work for the purpose of com
pliance with this Rule if precautions are taken to ensure his safety.
2. Shoring or timbering in excavation over 6.6 m. (20 ft.) deep and those installed to
prevent the movement, collapse of an adjacent structure shall be designed by a struc
tural engineer and approved by the proper authority.
5. Every excavation over 1 m. (3 ft.) shall be kept free of water at all times.
1. Tools or materials shall be kept a minimum of 1 m. (3 ft.) away from the edge of
the excavation to prevent their being knocked down into the excavation. 2. No vehicl e
or other machinery shall be driven, operated or located near the edge of an excav ation at
least a distance one-third (1/3) of its depth.
The top of the walls of an excavation more than 2.0 m. (6 ft.) deep shall be barricad
ed to a height of at least 1 m. (3 ft.) to prevent the fall of workers.
1. Every excavation over 1 m. (3 ft.) deep shall be provided with means of access an d
escape in case of flooding or collapse of the excavation work. 2. Every excavation shall have
at least one (1) ladder in every 16.6 m. (50 ft.) of length or fraction there of, of a length,
which shall extend at least 0.83 m. (2’6”) above the top of the excava tion to provide a firm
handhold when stepping on or off the ladder.
1. Timbering or support for any excavation shall be erected, added, altered or disman
tled only under the direction of the project supervisor.
2. Timbering and other support for any excavation shall be of good construction, sou
nd materials, and of adequate strength for the purpose for which it is used and prop
erly maintained.
3. All struts and bracings shall be properly secured to prevent displacement.
4. Timber giving off toxic saps or substance soluble in water shall not be used for ti
mbering.
1. When harmful dusts, gases and fumes are present in an excavation to such a degr ee
hazardous to the safety and health of the workers, all measures shall be taken eit her by
exhaust ventilation or by other means to free the area of such contaminants. 2. Internal
combustion engine shall only be operated in an excavation when provision is made to ensure
that the exhaust gases and fumes are rendered harmless or disch arged to a point away from
the excavation.
Daily checkpoints to match the state or condition of excavation will be discussed and
approved by the Safety / Security Manager so as to execute the works securely and
safely. Furnish, install and maintain suitable protection for safe working of excavations as
necessary for conditions encountered.
Strutting and bracing for trenches and excavations shall be provided when the Contra
ctor considers it is necessary to do so or whenever the condition of the earth is haz
ardous to workmen or structures.
No workers are to be allowed to work on any loose ground if the slope is too steep to
ensure a safe foothold.
No load, plant or equipment will be placed or moved near the edge of any excavati
on where it would be likely to cause collapse of the side of the excavation and to e
ndanger any person.
Where persons are working at different levels adequate means of protection will be p
rovided such as flooring to be provided to prevent person below from being struck by
tools or other falling object.
Every accessible part of an excavation where any person could fall, necessary protecti
on of adequate barrier will be provided.
The Contractor shall provide sufficient lighting facilities in executing the earthworks sa
fely and correctly in nighttime.
2. Excess construction members at the end of the working day must be brought down,
prior to leaving the working at height platform.
3. Small items for installation at height must be placed in sturdy containers, not along the
platform flooring.
4. Debris generated during work activities at height must be placed directly into sacks/bags,
to be hauled down periodically in easy-to-handle quantities, through rope and pulley, not
just drop down directly from the elevated platform.
5. Concrete chipping/demolition works at height shall commence only after proper netting
has been fully installed and inspected, and its surrounding vicinities properly cordoned off.
6. During scaffolding or formworks installation at height, the whole work area vicinity
must be cordoned-off, watchers stationed around the area to ward off unauthorized
workers.
The Contractor is aware that falls are a major hazard in any construction work. Adeq
uate supervision and careful planning will always be considered before any work impl
ementation. Proper preventive measure whichever practical will be applied to avoid in
cident caused by fall.
At any wall opening, gangways, elevated workplaces and other places where any pers
on may fall, proper guardrail and toe board for fencing must be provided. The mater ial
used shall be of sound materials and good construction.
All floors opening through which any person could fall should be safely guarded.
All floor covers over floor openings shall be of sound material and safely secured ag
ainst sliding, falling or lifting out to ensure people could walk on it safely.
In any case where protection is removed to allow passage of goods or any materials, it
should be replaced immediately.
Any wall opening through which any person or materials could possibly fall off shoul d
be protected by guardrails and toe boards.
The Contractor will take every reasonable and practicable precaution to prevent any I
ncident. Proper and suitable safety equipment and apparatus shall be provided purpo
sely for that intent and will be continuously reviewed in any circumstances where req
uired and applicable.
Only workers of physically and psychologically fit shall be considered to work on roof.
Sufficient and suitable crawling board shall be provided when work is done on steepl y
sloping roofs. It should be firmly secured in position as soon as practicable.
Wherever practical to prevent danger when work is done on steeply sloping roofs, a
parapet wall or adequate guard rails and toe board should be provided at the edge of
the roof or a catch platform or adequate safety nets should be provided or other method
which are practical.
Warning sign shall be provided to keep people off any part of roof, which cannot be
walked on.
No works shall be done during heavy rains, high velocity winds and typhoon. Materia ls
at this level shall be securely tied to permanent structures during bad weather.
Appropriate means and practical safety precautions shall be provided to keep people
off any part of the roof, which cannot be walked on.
Where possible and practical the pre-fabricated parts should be made so that they c an
be safely transported and erected.
Proper measures shall be considered to ensure the stability and condition of the part
when erected; the operation must be supervised by competent supervisor who shall
where practical review the safe operating procedures, which shall include:
2. Safe method of safeguards such as railings, working platforms and other means w
hichever practical.
3. Measures to ensure that the devices used are maintained to the requirements and
could stand the stresses to which they are used.
The store place condition must be thoroughly checked for stability to avoid circumsta
nces if possible that the prefabricated parts falling or overturning.
Instant review of work condition shall be reviewed during bad weather and possibly
no such work to be carried out in any violent weather.
Sufficient extended area shall be provided for the steel parts to be erected and barri
caded.
Lifting part should be checked for any material that could possibly fall down before t
hey are lifted and they must be properly fastened.
No workers shall be allowed to stand under load, which could possibly injure by falli
ng.
A proper control of people in the working area is compulsory. Signaling and radio wi ll
be used as means of communication between the working levels and the ground.
Hand ropes should be used to control the load where any panel or structural memb
ers are being hoisted.
Proper warning signs shall be erected to warn affected workers of the possible falling
hazard.
Main gases involved in gas welding contain considerable chemical and chemical ener gy
when compressed. It shall be used in a controlled manner otherwise it could caus e disaster.
The gases shall not be allowed to leak into the atmosphere. Leak testing of hoses an d
fitting must be carried out at a regular basis using water or foam.
The regulators must always be checked to confirm that they are in good condition.
No hot work shall be allowed unless they are equipped with flashback arrester for re
pair.
The use of pipe wrenches or pipe extension to increase leverage must be banned.
Joint and screw threads should be kept free from grit, as the abrasion caused can im
pair the gas seal efficiency, which inevitably leads to the use of excess tightening of

All welders shall wear eye and body protective gears during welding works.
24.2 Requirements for Proper Handling And Storing of Paints, Solvents And Chem
ical Agent
Improper storage and use of chemicals may endanger health and cause undue dama
ge to property. Chemicals shall be stored in suitable containers in appropriate locatio ns
and must not be stored together with any other mutually reactive materials. Toxic, corrosive
or other hazardous materials should be treated with great care as mentione d hereinafter:
24.2.1 Handling
Containers of flammable liquids shall be properly handled to ensure that spillage can
not occur when they are moved.
No smoking must be allowed at any time while handling such chemical (paints, solve nt,
and chemicals).
Flammable material should not be kept in the vicinity of any hot work, excessive hea t
or non-intrinsically safe electrical apparatus.
Over exposure through inhalation of the chemical may result either to Accouter Chro
nic poisoning. Proper chemical respirator should be worn while handling such toxic
chemical.
Skin contact with liquid may lead to irritation. Immediate action by flushing the eff
ected area for about 25 - 50 min may reduce the effect. Obtain medical assistance I
mmediately.
Full range of protective clothing should be worn (respirators, goggles, gloves) to avo id
personnel contact with the chemical.
Personnel handling the chemicals shall be those with sufficient knowledge of the asso
ciated hazards and they shall be informed of the whereabouts of the nearest first-aid kit,
running water or fire extinguishers.
Adequate measure should be taken to avoid any spillage of chemical. Chemicals spill
age must not at any time be disposed into any drain, watercourse or whatever which
could contaminate the environment.

24.2.2 Storage
Amounts of flammable liquid kept at work site shall be limited to minimum to preve nt
spillage when they are moved.
“No smoking” signs must be strategically placed. Matches and cigarettes shall be con
trolled and avoided in chemical storage area.
Flammable materials must not be stored together with any combustible material; it sh
ould be placed well away from stock of flammable liquids.
All paints, solvents, chemicals must be properly labeled with following information:
1. Name of material;
Only appropriate amount of chemical will be allowed to be taken out from stores tha t
are sufficient for the job. Sufficient number of extinguishers with signage indicator s shall be
placed within the store area.
No food shall be consumed in or taken where chemicals and solvent are handled.
Protective equipment shall be worn when instructed or if laid down in the procedure.
4. A record of explosives received and used shall be properly maintained and open f or
inspection by the enforcing authority.
25.2 Blasting
1. Only the quantity of dynamite required for immediate use in blasting shall be rem
oved from the magazine.
b. within 6.6 m. (20 ft.) of a hole being loaded with explosives or blasting agents.
3. Every firing circuit in connection with blasting operations shall be broken in a suita
ble manner at a safe distance from the blasting area.
4. When a charge is fired, steps shall be taken to see that persons employed are in a
position free from the explosives or from lying objects.
Before a worker or group of workers enter any confined or enclosed space and befor e
any work is commenced, the following precautions and safety measures shall be ta ken:
1. The area shall be checked visually to ensure that the water level is below 15 cm. (6 in.)
and if water is present, a dry wooden platform shall be available for use;
3. If any of the above is present over normal levels, the area shall not be entered until
ventilation by blower is affected.
4. Approved types of breathing apparatus and other personal protective equipment shall
be provided and made available for use by the worker or workers entering a confined

5. No worker or group of workers shall enter a confined space unless a watcher is available
who is familiar with the job and in contact with the men at regular intervalsand equally
provided with breathing apparatus for ready use in case of emergency.
8. Any manhole, tank opening or other opening which is left unattended should be
protected during the day by barricades, and at night by barricades and lanterns, with
appropriate warning signs; and 9. Adequate means of ingress and egress from any co
nfined or enclosed space shall be provided.

- providing certified safe drinking waters to all workers, and identifying with signage’s
other water supply sources not fit for drinking;
- preventing the formation of stagnant pools of water in the vicinity of housing and
office accommodation.

- prompt and effective collection and disposal of all domestic and industrial waste ge
nerated on or about the Site.
To prevent the formation of any stagnant water pools around the vicinity of the hous
ing and office areas, the following measures shall be undertaken, as the exigencies re
quire:
2. In areas where excavation of open drainage trench is not feasible, portable submersible
pumps shall be used to drain stagnant water into the nearest temporary drainage system.
3. When any of the above measures cannot be done immediately due to some unav
oidable circumstances, such stagnant water shall be treated with required biodegrada
ble chemicals to avert the breeding of virus carrying insects and other water insects.
To ensure full control and elimination of unwanted weeds, shrubs, insects and other
noxious matter within the project site, the Donggwang shall maintain a regular site m
aintenance crew to concentrate on the overall maintenance of the whole construction
site area, both within the building and the entire external jobsite premises. Such mai
ntenance crew shall be directly under the supervision of the Safety-Security section a s
a support team for all unwanted site situations requiring immediate remedial action.
To maintain all temporary and permanent structures within the project site free of illn
ess carrying insects and other pest, there shall be undertaken a periodic spraying of
bio-degradable, non-toxic chemicals in all such structures – the frequency of which w
ould depend on the magnitude of infestation.
2. The prospective garbage collector for each garbage category shall be required to include
the following documents in their proposal:
a. Formal letter of approval from the land owner for the dump Site
b. Formal letter of approval from the City’s Environmental Department for the dumpin g
of garbage at the proposed dump Site.
c. Formal clearance from the Barangay concerned where the dump Site is located.
d. Required garbage dump truck with proper canvas cover to ensure litterfree hauling
from jobsite to dump Site.

FilHigh shall be responsible for the control of visitors to site and shall take such prec
autions as are necessary for their safety as may be agreed with the Engineer. Records of
all visitors to the site will be in a form to be agreed by the Engineer and shall su bm it
details of all visitors to the Engineer in a weekly basis.
The purpose of this policy is to provide a uniform basis for the presentation of safet y
awards.
Safety / Security Manager shall keep records of every contractor performing on works ite
for their safety performance. He shall maintain a proper record and formulate a p oints
system. Contractor with fewest safety violations will be rewarded.
FilHigh will administer necessary discipline in a fair, just and reasonable manner. To
establish subcontractors and workforce that willingly adheres to and implements the s ite
safety rules and regulations and the provisions of the comprehensive Safety Progr am to
achieve a cooperative safe working environment.
As stated earlier on, all penalties imposed, whether on the subcontractor or on his s
upervisor or his own workers, shall automatically become deductible from the periodi c
billing to the FilHigh of such subcontractors.

To ensure the safety of both the drivers/operators and the numerous personnel movi
ng around the jobsite, the following basic guidelines shall be strictly implemented.
1. The mandatory speed limit of 20 km. within the whole jobsite shall be strictly
implemented; with road signage’s installed at entry gates and all over the roadways within
the project site.
2. Violators shall have their site I.D, confiscated, brought to security headquarters for
reprimand, briefing, peso penalties and given final warning. Second violation shall m
ean “Ban Entry” into the jobsite.
3. This policy shall apply to both road vehicles and equipments within the jobsite.
4. Parking of vehicles and equipment shall be strictly limited to designated areas insi
de the project site. Those parked or stalled along construction pathways and roadway s
shall be towed to designated area – towing fees and penalties deductible from con cerned
subcontractor’s billing;
5. No major repairs of vehicles/equipment are allowed within the project site, to prot
ect the site from pollution from spillage of oil and fuel, and discarded defective junk
parts. When the situation is unavoidable, required protection from pollution, and prop er
board-up walling should be provided. 6. Drivers/operators shall be fully briefed on these rules
and regulations on movement of vehicles/equipments within the project si te – prior to

31.0 Housekeeping
1. Daily clean up of scrap material, sawdust, rags, oil, grease and other residue of
construction operations shall be performed.
2. Pallets, blocks, cribbing, dunnage, or similar materials shall be stored clear or walkways
in an organized manner when not in use.
3. Materials shall be stacked, blocked, interlocked, and limited in height so that it is stable
and secured against sliding or collapse.
4. Lay down, parking, and temporary facility areas shall be maintained in a neat and orderly

manner.
5. Hoses, cords and welding leads shall be maintained in an orderly fashion and clear of
walkways, ladder ways, stairways and other access ways. If they are no longer in use, roll
them up and place them out of the way.
6. Material shall not be stored on scaffolds or runways in excess of needs for normal
placement operations, or in excess of safe load limits.
7. Material shall not be placed near floor openings or stacked in areas where it can be
dislodged by vibration.
8. Tools or material shall not be stacked above scaffold or platform toeboards, mesh, or
bulkheads.
9. All stairways, passageways, gangways, and access ways shall be kept free of materials,
supplies and obstructions at all times.
10. Protruding nails in scrap boards, planks, and timbers shall be removed, hammered in,
or bent over flush with the wood.
