This affidavit of the witness, Dr. Jaime Tuazon, M.D. is being offered to
prove the truthfulness and veracity of the medical report marked as exhibit
___________________________________.

JUDICIAL AFFIDAVIT OF DR. JAIME TUAZON, M.D.

I, DR. JAIME TUAZON, M.D., of legal age, Filipino, married, resident of

Manga Dist., Tagbilaran City, after having been duly sworn to in accordance
with law, fully conscious that I am answering the following questions under
oath, and that I may face criminal liability for false testimony or perjury and
said judicial affidavit was conducted at the Law Office of Att. Jean Isabelo
Tubac, located at CPG Avenue, Corner Visarra St., Tagbilaran City, which
taking was done in English which was also explained in a dialect known to me
and that the other information or requirements of Sec, 3, A.M. 12-8-8-SC as
explained to me by the administering officer, hereby deposes and state as
follows:

DIRECT EXAMINATION QUESTIONS:

Question (Q): Please state your personal circumstances.

Answer (A): I am Dr. Jaime Tuazon, M.D., 50 years of age, Filipino, married and
a resident of Brgy. Poblacion, Dauis, Bohol, a medical practitioner for the
Province of Bohol.
Q: How long have you been practicing your profession?

A: For almost 15 years already.

Q: Where are you presently connected?

A: At the Governor Celestino Gallares Memorial Hospital, Tagbilaran City.

Q: what position do you currently hold?

A: I am the Senior Medical Examiner in the said hospital.

Q: Have you examined victims of domestic violence before?

A: Yes Ma’am.

Q: Do you know the accused Lucy Felias in this case?

A: Yeas ma’am. I have examined Lucy Felias after she was brought to the Police
Station for allegedly killing her live-in partner. I was called by Atty. Jean
Isabelo Tubac, her counsel, to conduct an examination for her claim that she
was domestically abused.

Q: Could you describe the physical built?

A: She was thin, her stomach was big for she was six months pregnant, 5 feet
and 3 inches in height, and of fair complexion.

Q: What was your finding after conducting a medical examination of her?

A: After examining her, she had a first degree burn on her face about 6 inches
long and 5 inches wide from her forehead down to her chin. She also suffered
contusion on her head 5 cm wide. The burn and contusion is about 1 day old. I
also found out that she had bruises in her legs, arm, back, and at the side of
her abdomen about 3 days old, minor scratches in her arms and legs and a slit
on her lips about 3 cm long.

Q: As an expert witness, can you confirm that the accused Lucy Felias is a
victim of domestic violence?

A: Yes. Based on my findings, the bruises and scratches in her arms and legs,
and back was already 3 days old and is already healing. Therefore, there was
an abuse made prior to the incident that took place last night.

Q: did you prepare a medical report showing such findings?

A: Yes, ma’am.

Q: I have here a document from GCG Memorial Hospital entitled Medical

Certificate dated October 16, 2019, is this familiar?

A: Yes ma’am. That is the medical report I issued for Lucy Felias.

Q: At the bottom of this document, there appears to be a name Dr. Jaime

Tuazon with a signature over it, can you confirm whose signature is this?

A: yes ma’am, that is my signature.
