1. In case of treatment at the registered hospital (“Registered

Hospital” means the hospital in which the insured selects as the

only hospital where the insured would be treated except that the

treatment is urgently needed such as emergency case)

 Submit the application along with relevant documents to the

Provincial Social Security Office.


3. Reimbursement via bank transfer
a. Complete SorPorSor 2–01 or SorPorSor 2–16 and attach

supporting documents.
b. Submit to the administrative office of the staff member’s unit.

The administrative office will submit the documents to the

HR division every 15 and 30 day of each month. Should the

dates fall on a holiday, the documents will be submitted to

HR division the next working day.


c. The HR division checks the documents and submits them to

the Social Security Office.


d. The Social Security Office checks the documents and remits

the reimbursement to the insured’s account.

Entitled Benefits

1. In the event of injury or sickness


The insured shall have made 3 full months of contributions

before receiving medical treatment.

Guidelines and Entitlement Benefits

General sickness: The actual incurred outpatient expenses are

reimbursed.

In the case of emergency care

For critical sickness which necessitates prompt medical attention

or situations in which the life of the insured may be put at risk, the

insured may be admitted to the nearest hospital. The insured or a

relative of the insured or a concerning person must report to the

hospital associated with the Certified Entitlements Card as soon as

possible so that the hospital may take responsibility for the medical

expenses. The district Social Security Office shall take responsibility

for the expenses incurred for up to three days (72 hours) prior to

reporting to the hospital associated with the Certified Entitlements

Card, according to the category and the announced rates.

 For out-patient care, reimbursement of the medical bill can

be made as actually incurred, but not exceeding 300 Baht per

instance.
 For in-patient care, reimbursement of the medical bill can be

made as actually incurred within the first 72 hours, but not

exceeding 1,500 Baht per instance. Note that the cost of

ward room and food may be reimbursed at a rate of not

more than 700 Baht per day. If treatment is received in a

private hospital and an ambulance is used, the cost of the

ambulance service may be reimbursed up to 500 Baht per

instance.

**In the case of injury, the insured may apply for reimbursement of

the medical expenses without limitation to the number of admissions.

5. For reimbursement by bank transfer, a copy of the first page

of the Bank Savings Account book on which the Account

Name and the Account Number belonging to the Insured are

shown. Payment may be made by way of 8 commercial

banks: Krung Thai Bank PLC; Ayudhya Bank PLC; Bangkok Bank

PLC; Siam Commercial Bank PLC; Kasikorn Bank PLC; The Thai

Military Bank PLC; Thanachart Bank PLC and the Islamic Bank

of Thailand.

In case of dental treatment (extracting, filling, removing

dental plaque and fitting of denture)

Guidelines and Entitlement Benefits

The insured is entitled to receive dental care at any hospital in

the case of extraction of teeth, filling teeth, removal of dental plaque.

The cost may be reimbursed not exceeding 300 Baht per each dental

treatment or 600 Baht per year.

For partial over denture, the insured shall further be entitled to

be fitted with an acrylic-based over denture within a period of 5 years

with effect from the date of fitting the acrylic-based over denture as

follows:

a) 1 to 5 teeth at a cost not exceeding 1,300 Baht.

b) an over denture for more than 5 teeth, at a cost not

exceeding 1,500 Baht

For upper and/or lower denture, the insured is entitled to

reimburse the cost of service as actually incurred not exceeding 4,400

Baht within 5 years, with effect from the date of fitting the acrylic-

based over denture as follows:

a) Upper OR lower over denture: as actually incurred not

exceeding 2,400 Baht.

b) Full (Upper and lower) over denture: as actually incurred not

exceeding 4,400 Baht.

Documents required to apply for dental service cost

1. Application Form for Compensation Payment in Case of Dental

Care (Sor. Por. Sor. 2-16).

6. For reimbursement by bank transfer, a copy of the first page of

2. Maternity Benefits
The insured shall have made a full 7 months of contributions

within the 15 months preceding the month of delivery.

Guidelines and Entitlement Benefits


 Childbirth may be given anywhere. Reimbursement shall be on a

lump sum basis at the rate of 13,000 Baht per childbirth.


 Female personnel are entitled to 90 days of paid maternity

leave at a rate of 50% their average salary.

Remark The record of applying for childbirth benefits will be checked.

The insured (husband and wife) may not make use of this entitlement

more than a total of 4 times. In addition, the entitlement may be

used only once per child.

Documents required to apply for the benefit

2. Original Birth Certificate and a copy of the Birth Certificate
3. Copy of Work Permit and Passport of the father and the mother
4. Copy of Marriage Certificate or the insured certificate (In case the

marriage is not legally registered)


5. To receive the benefit payment through bank transfer, a copy of

the first page of the Bank Savings Account book on which the

Account Name and the Account Number belonging to the

Insured are shown. Payment may be made by way of 8

commercial banks: Krung Thai Bank PLC; Ayudhya Bank PLC;

Bangkok Bank PLC; Siam Commercial Bank PLC; Kasikorn Bank

PLC; The Thai Military Bank PLC; Thanachart Bank PLC and the

Islamic Bank of Thailand.

3. Disability Benefits (Non-work related)


The insured shall have made a full 3 months of contributions

within the 15 months preceding the month in which the Social

Security department declares the insured to be disabled.

Guidelines and Entitlement Benefits
If care is received in a government hospital, out-patient and in-

patient care are free of charge.


If care is received in a private hospital, the cost of out-patient

care will be reimbursed as actually incurred but not exceeding 2,000

Baht per month and the cost of in-patient care will be reimbursed as

actually incurred but not exceeding 4,000 Baht per month.


Cost of transportation for the disabled: 500 Baht per month.
The insured is entitled to reimburse the cost of rehabilitation in

a Social Security rehabilitation center up to 40,000 Baht.

4. Death Benefits (Non-working related)


 The insured shall have made one full month of contributions

within the six months prior to the month of death.

Guidelines and Entitlement Benefits


 The funeral benefit of 40,000 Baht payable to the person

responsible for the funeral.


 Assistance benefit in the event of death as follows:
o In the case of contributions for upward of 3 years but less

than 10 years, the insured shall be entitled to an assistance

benefit equal to 1.5 months of his/her average wage.


o In the case of contributions for upward of 10 years, the

insured shall be entitled to an assistance benefit equal to 5

months of his/her average wage.

Documents required to apply for reimbursement


1. Form SorPorSor. 2-01 completed by the person responsible

for the funeral who is entitled

he/she is responsible for the funeral

A letter of power of attorney
The original copy of Passport of the grantor of authorization and

of the authorized person


A letter notifying the result of the review from the district Social

Security Office
For convenience, the application for collection of the funeral benefit

may be submitted to the Social Security Office in any district or

province in which an office is located.

The insured shall have made a full 12 months of contributions

within the 36 months preceding the month in which the insured is

entitled to the benefit.


 The child assistance benefit is paid on a lump sum basis at the

rate of 400 Baht per month per child.


 The child assistance benefit for a lawful child aged not more

than 6 years shall be limited to not more than 2 children.

(Adopted children and children given for adoption are excluded.)


 If an insured male wishes to receive benefits for a child from an

unregistered marriage, he may follow one of three approaches:


o To proceed with registration of marriage with the child 's

mother
o To legally register as the child’s father
o To file a petition with the court and obtain a court order

stating that the child is the insured’s child.


The entitlement will cease under any of the following conditions:
 The child reaches the 6th year of age
 The insured’s Social Security insurance terminates
 The child is deceased
 The child is adopted.

3. A copy of the spouse 's Citizenship Identity Card or Passport
4. A copy of the Certificate of Marriage/the Certificate of Divorce of

the Insured (in case of divorce registration).


5. To receive the benefit payment through bank transfer, a copy of

the first page of the Bank Savings Account book on which the

Account Name and the Account Number belonging to the

Insured are shown. Payment may be made by way of 8

commercial banks: Krung Thai Bank PLC; Ayudhya Bank PLC;

Bangkok Bank PLC; Siam Commercial Bank PLC; Kasikorn Bank

PLC; The Thai Military Bank PLC; Thanachart Bank PLC and the

6. Pension Benefits
Conditions for Pension Entitlement
1. The applicant shall have made contributions for 180 full

months, not necessarily consecutive.


2. The applicant shall have reached the age of 55 (Retirement age

for private sector employee)


3. The status of being an insured shall have terminated.

 The insured is entitled to a monthly pension at the rate of 20

percent the average monthly wage over the last 60 months of

employment.
 In the case of making contributions for over 180 months, the rate

of old age pension benefit pursuant to the condition above shall

be increased at the rate of 1.5 percent for every 12 months.

Guidelines and Entitlement Benefits (Gratuity Benefit)


 In the case of making contributions for less than 12 months, a

gratuity benefit shall be paid equal to the amount of the

contribution made by the Insured.


 In the case of making contributions for greater than 12 months, a

gratuity benefit shall be paid equal to the amount of the

contribution made by both the Insured and the employer.

Documents required to apply for pension benefit

1. The Application Form for collection of the old age benefits

(Form SorPorSor. 2-01)

2. A copy of the Passport and Work Permit

3. To receive the benefit payment through bank transfer, a copy of

the first page of the Bank Savings Account book on which the

Account Name and the Account Number belonging to the

Insured are shown. Payment may be made by way of 8

7. Unemployment Benefits
 The insured shall have made a full 6 months of contributions

within the 15 months preceding the unemployment. The

unemployment may not be associated with any violation of the

law.

Guidelines and Entitlement Benefits


In case of termination of employment
o Unemployment benefit for not more 180 days per year at the

rate of 50% of wages.


In case of resignation or end of contract
o Unemployment benefit for not more than 90 days per year at

the rate of 30 percent of wages.


Unemployment benefits shall terminate under any of the

following circumstances:
o Assume new employment.
o Refusal to accept a position or job training that has been

arranged.

o Failure to report as required.

Documents required to apply for unemployment benefits

1. Application Form for Unemployment Benefits (SorPorSor. 2-01/7)

2. Register as unemployed at the Office of Employment, the

Department of Employment.

3. The Passport and a copy

4. One photograph

5. Letter of Termination of Employment or a copy of the Notice of

Leaving Employment (SorPorSor.6-09) or a letter or an order

from the employer terminating the employment (if any).

6. To receive the benefit payment through bank transfer, a copy of

the first page of the Bank Savings Account book on which the

Account Name and the Account Number belonging to the

Insured are shown. Payment may be made by way of 8
