Description of the Learner

The patient is Puddunan, Dorris joyce, 25 yrs old. She was admitted because of RLQ pain. She is on s/p explore laparotomy and
appendectomy. The patient is a commerce student at BCU. After the operation the patient is complaining of incision pain. She doesn’t do
anthing to relieve it according to her.

Learning need
The patient is in need of information regarding post operative care activities.

Educational Diagnosis
Knowledge Deficit: Information about her condition related to lack of exposure to information as manifested by patient asking questions
about her condition.
Goal
The patient will acquire Information about her case.

SIGNS AND SYMPTOMS OF WOUND INFECTION
· Redness, fever and pain
· Tenderness
· Purulent drainage/edema
· Wound separation/wound odor

HOW TO PREVENT WOUND INFECTION


Good nutrition especially foods rich in Vitamin C-promotes collagen formation
Keep wound dry and clean.
Refrain handling wound with dirty hands- do proper hand washing.
Proper cleansing and applying dressings
Disposed used wound dressings properly.
Seek medical attention if signs and symptoms of wound infection is observed to prevent further infection.
Maintain a good hygiene.
MATERIALS FOR WOUND CLEANSING AND BANDAGING
1. Cleansing solution- Hydrogen Peroxide and Betadine solution (most prescribed); has an antiseptic effect and
prevents irritation of the wound.
2. Bandage materials-gauze, surgical tape or alternative materials such as clean cloth.
3. cottons- dry and cotton soaked in hydrogen peroxide and betadine
4. Scissor
5. Wound dressing- protects the incision from outside injury
- absorbs drainage of exudates and secretions from the wound
- maintains moist environment that promotes wound healing

STEPS IN CLEANSING AND BANDAGING THE WOUND

1. Perform hand washing.


2. Prepare needed cleansing materials (hydrogen peroxide and betadine)
3. Prepare gauze or sterile dressing and cotton balls.
4. Remove dressing by removing the adhesive tapes carefully
5. Look for signs of infection.
6. Gently clean the wound area starting from the wound site going farther. First use hydrogen peroxide to
soften encrustinations then apply the betadine solution in the same manner (from the suture side to the
surrounding areas).
- You can use dampening motion while cleaning the wound site and circular motion when cleaning the
outer area. Remember no to re-use cotton balls applied to the same area.

7. Cover wound with a dry dressing. Depending on the size of the wound is the amount of dressing used. Layer
dressing is applied to provide wound with adequate oxygenation.
8. Apply adhesive over the layered dressing following a single direction.
9. Avoid wetting the wound with water during bathing. Keep wound dry at all times and maintain an intact
dressing.
10. Clean wound at least once or twice a day to prevent infection and to promote wound healing.
11. Return for follow-up check up if necessary.

1. DON’T RESUME TO HEAVY ACTIVITIES, TRY TO REST AND RELAX
2. AVOID STRESS AND STRAINING
3. DON’T TOUCH WOUND WITHOUT WASHING HANDS
