ARTICLE I. PRELIMINARIES:
Section 1. Published hereunder is the General Guidelines and Policies regarding the Industry Immersion Program of Cabangan
High School for Senior High School students.
Section 1. All student- trainees must accomplish all forms and information sheets required in the work immersion portfolio.
Section 2. The student-trainees must submit all requirements required by the School and the partner Industry for work
immersion.
Section 3. After the Industry Immersion, the student- trainees should submit the Industry Immersion portfolio duly
accomplished and approved together with the following:
a. Company Profile
b. Rules and Regulations set by the partner-industry
c. Evaluation Sheets of the Training
Section 4. The above requirements should be submitted typewritten or computer type, double spaced on a long bond paper
(8.5” x 13”) and shall be made as Part 2 of this Immersion Portfolio.
Section 5. The Immersion Portfolio must be submitted and approved one (1) week after the completion of the Training.
Section 6. All student-trainees are expected to follow all company rules and regulations, and must observe punctuality and
safety in their work areas at all times.
Section 7. Every student-trainee should stay away from areas/places designated as RESTRICTED or DANGER AREAS
and neither to horseplay nor tinker with equipment.
Section 8. Consult the Teacher-in-Charge of your Specialization, the Partnership Focal Person, or the School Head on any
incident which occurred or any decision with regards to the immersion.
Section 9. Student-trainees should foster good relationship with their superiors and fellow trainees.
Section 10. The following are forms to be accomplished by each student-trainee and used by the School with regards to the
Industry Immersion Program:
a. Form 01: Trainee’s Information Sheet
b. Form 02: Industry Immersion Health Form
c. Form 03: Letter of Endorsement
d. Form 04: Parent’s Consent Form
e. Form 05: Company Acceptance Form
f. Form 06: Daily Log of Activities and Training Hours
g. Form 07: Trainee’s Attitude and Evaluation Form
h. Form 08: Technical Skills Evaluation Form
i. Form 9: Rating Sheet Summary
j. Form 10: Trainee’s Self-Assessment
k. Form 11: Industry Immersion Clearance and Certification
l. Form 12: Industry Immersion Handbook Approval Sheet
m. Form 13: Company Information Sheet
ARTICLE III. INDUSTRY IMMERSION GUIDELINES AND POLICIES
Section 1. The Industry Immersion is an integral part of the Senior High School. The student- trainees and the SCHOOL’s
designated Partnership Focal Person shall abide by the company’s rules and regulations and comply with those
imposed for the industry immersion; otherwise they shall be excluded from further participation;
Section 2. Student-trainees must submit themselves to any immersion program’s preparatory activities like meetings, seminar
on work ethics, orientations, and the like.
Section 3. Only those with completed required academic course subjects can proceed to industry immersion program and shall
be identified by his/her Class Adviser and endorsed by the School.
Section 4. Failure to complete the required training hours as agreed by the School and the Industry disqualifies the student for
graduation.
Section 5. There should be no labor management relationship between the student- trainee and the Company’s personnel.
Section 6. The student-trainees will be responsible for all liabilities for damage to property or injury to third person which may
be occasioned by their intentional or negligent acts while in the course of the industry immersion;
Section 7. The student-trainees are not entitled to a job at the conclusion of the training period and are free to take jobs
elsewhere in the same field; however, the COMPANY may absorb a student-trainee after graduation.
Section 8. The student- trainee renounces and waives his/her claim against the industry immersion cooperating agency for any
loss that may suffer personally monetary in the performance of his/her duties or functions while under the training.
Section 9. All student-trainees should understand that all information on technology, manufacturing process, process standards,
quality assurance methodologies, quality standards, production capabilities, raw material purchasing, marketing,
finance and all other related documents, manuals, operational and technical matters that the Company shall make
available to them shall be used for the sole purpose of student training. All of these matters are classified as
confidential in nature and proprietary to the Company and thereby each student undertakes to prevent transfer of
such information by any of its members to any party outside the Company.
