    This is relative to your inquiry re: Application for One-Time Financial Assistance under COVID-19
    Adjustment Measures Program (CAMP).

    Please be guided of the following:

    Applicability: If company/establishment is implementing Flexible Work Arrangement or Temporary


    Closure where there are:

    1. Retained workers who do not receive regular wage - whose working hours is
    reduced and therefore, regular wage is reduced due to the implementation
    of Flexible Work Arrangements as mitigating measures enforced
    by the employer;
    2. Suspended workers - workers whose employment is temporarily suspended by
    reason of the suspension of operations of the employer's
    business establishment; and

    Documentary requirements: Application for One-Time Financial Assistance (P5,000.00)

    1. Completely filled-up and duly signed Establishment Report on the COVID-


    19 (forms downloadable at ro11.dole.gov.ph);
    2. Company Payroll for the month of February or earlier;
    3. List of Affected Workers in Word and Excel Format (Excel format is an
    additional requirement for easy processing of payment/assistance )

    Please take note that the application may be denied by the DOLE Regional Office upon determination of
    any of the following grounds:

    1. Ineligibility of applicant;
    2. Misrepresentation of facts in the application; or
    3. Submission of falsified or tampered document.

    1. Labor Advisory No.11-20 Supplemental Guidelines relative to the Remedial


    Measures in view of the ongoing outbreak of Coronavirus disease 2019 COVID-
    19;
    2. Labor Advisory No.-09-20 Guidelines on the Implementation of Flexible Work
    Arrangements as Remedial Measure due to the Ongoing Outbreak of
    Coronavirus Disease 2019 COVID-19;
    3. Department Order No. 209 Guidelines on the Adjustment Measures Program for
    Affected Workers Due to the Coronavirus Disease 2019
    4. Labor Advisory No. 12 - Clarificatory Guidelines on the COVID-19 Adjustment
    Measures Program (CAMP)

    required documents through electronic mail ONLY. No personal applications in all DOLE Offices shall
    be entertained.
