1. BACC Form No. 02, duly accomplished in three (3) copies and properly notarized
with metered documentary stamp in the originalcopy,
Photocopy of expired Certificate of Registration/Accreditration to practice public
accountiancy

Photo copy of current professional identification card(s) of individual CPA, and

partners for partnerships,

.4. Certified copies of all present business permits, cunent professional tax receipts
(PTR) of all CPAs staff and oartners issued by the local and /or national
Government.

5. Certification of Continuing Professional Education (CPE) Units Earned from

a duly accredited CPD Providers, CPD Council for Accountancy.
