Course Title: Care of Mother and Child at Risks or with Problems (Acute and Chronic)

Course Description:
This course deals with the application of knowledge, skills, and attitude in providing safe, quality
and holistic care on at-risk/high risk clients during different childbearing stages, including
newborn and children up to adolescents at various developmental stages in the hospital and
community settings.

Level Outcome:
At the end of the 2nd year, given a normal and high risk mother, child, and family in any health
setting, the students will be able to demonstrate safe, appropriate, and holistic care utilizing the
nursing process.

Course Outcomes:
Given actual or simulated situations or conditions involving high risk pregnant women, women
during labor and delivery, postpartum women, neonates, infants, toddlers, preschoolers, school age
children, and adolescents, the student will be able to:
1. Apply concepts, theories, and principles of maternal and child nursing in the formulation and
application of appropriate nursing care to high risk mothers and children.
2. Perform complete and holistic assessment of a couple with reproductive problems; a high
risk mother during pregnancy, labor and delivery, and postpartum; a high risk newborn; and a
child with physiologic problem.
3. Implement safe and appropriate nursing interventions addressing the health care needs of a
high risk mothers and children.
4. Utilize evidence-based nursing practices when caring for high risk mothers and children.
5. Provide health education on maternal and child health using appropriate teaching strategies.
6. Evaluate with mothers and children the progress of their condition and outcomes of nursing
care rendered.
7. Ensure a well-organized, complete, and correct documentation of maternal and child’s
responses to nursing care.
8. Collaborate with the health care team for effective and efficient maternal and child care.

A. Group Activity 1: Submission on Week 8


This is a group activity aimed to integrate concepts of high-risk mothers and reproductive health problems. Total is
100 points. Print your work using Times New Roman font, size 12, in short- size, one-sided, single-spaced bond paper,
paginated on the right bottom page, with 1-inch margin on all sides, and place in a short sliding folder. Submit with a
cover page listing all the members of the group. Do not forget to include the references at the last page following the
APA format. Now, do what is asked and answer all the following questions. For items 1 to 5, all members of the group
must have a narrative entry for each and a synthesis of the answers must be provided in the end.
1. When is pregnancy considered to be a high risk one? Recall what you have learned about fetal
development and identify all possible maternal factors you could think of which can affect
both maternal and fetal health. Would the presence of these risk factors make the occurrence
of maternal and fetal health problems highly probable? (15 points)

5. Christmas, 36 years old, G3P3, is suffering from postpartum hemorrhage due to uterine atony.
She gave birth 4 days ago to a 38-week baby girl via vaginal delivery with right mediolateral
episiotomy. What possible factors possibly contributed to the postpartum hemorrhage? What
priority interventions will you consider when caring for this woman? Include the concepts of
EINC and SDG in your discussion. (15 points)

6. Look for a woman who is or has been diagnosed with any of the female neoplasias we have
discussed. Interview this woman and make a short narrative summary of all her experiences
associated with the diagnosis, including both physiologic, psychologic, sociologic, and other
concerns. All members of the group must be present during the interview so it is important that
your chosen client agrees to this. Submit proofs that the group did the interview. You may take
photos and attach it in this item. Please ask consent when doing so. Take note that
confidentiality and privacy of clients are most important. Do not show the woman’s face and
no need to mention names. (25 points)

This is a group activity aimed to integrate concepts of high-risk newborns and children with physiologic disorders.
Total is 100 points. Print your work using Times New Roman font, size 12, in short- size, one-sided, single-spaced

1. Make a narrative analysis of the priority nursing care on basic physiologic needs of a high-risk
newborns who is 7 months premature with respiratory distress syndrome and from a drug-
dependent adolescent mother. Include these needs as oxygenation, fluid and electrolyte,
temperature regulation, nutrition, elimination. How do you think should a nurse provide
developmental care to this newborn confined in an intensive care unit. Include how you can
advocate mother and child bonding even at this stressful condition. (25 points)

3. Faith is a 4-year-old girl who has been brought to the clinic by her parents due to three episodes
of coughing and wheezing, with the first episode occurring approximately 6 months
previously. The child has a history of eczema and her mother suffered from asthma. The father
is a smoker. To consider this as a case of asthma, what signs and symptoms are present in the
child? What other symptoms not seen in this child may be expected in asthma? What is the
cause of this condition and how is such a case managed in general practice? (25 points)

4. Baby Hope, a 3-week-old infant, was admitted due to severe central cyanosis caused by several
congenital heart problems. Soon after birth, he suffered from respiratory distress, where his
initial oxygen saturation was only about 70%. He was resuscitated and given a prostaglandin.
On appearance, he was dusky-looking and his peripheries were cold and blue. Antenatal scans
and cardiac catheterization both found pulmonary stenosis, overriding aorta, and ventricular
septal defect (VSD). What specific congenital heart condition does this baby have? Briefly
discuss its pathophysiology. What interventions would be implemented to address the baby’s
general health? Include activities to prevent possible complications that may arise. As the nurse
caring for the baby, how would you explain the situation to the neonate’s parents? How would
you include them in the plan of care? (25 points)
