PRELIMINARY STATEMENT

This Judicial Affidavit was taken at the office of Atty. ATTOR NEY at
123 ABC St., Sta. Mesa, Manila City. The Petitioners answered the questions
under oath and with full knowledge that failure to answer truthfully may hold
them liable for false testimony and perjury.

OFFER OF TESTIMONY

This Judicial Affidavit of the Petitioners is executed to serve as their

direct testimony in the instant case.

This Judicial Affidavit is being offered:

 1. To prove the allegations in the petition, including all annexes
appended thereto and marked as exhibits;

2. To refute all the material allegations in the pleadings submitted by

the other party, including all the annexes therein; and,

3. To prove all other related matters, facts, and circumstances, and

documents which are relevant and material to this case.

EXAMINATION OF THE WITNESS

Questions were propounded by Atty. ATTOR NEY and are numbered

consecutively. Each question is followed by the answer of the petitioners.

1. Do you swear to tell me the truth and nothing but the truth?
I do.

2. Are you aware that you may face criminal liability for false
testimony or perjury if you will not tell the truth?
I am.

3. Please state your name, age, address, and occupation.

I am JBS, 25 years old, married, residing at 345 BEC St., Sta. Mesa,
Manila, working as a software developer.

4. Are you the petitioner in this case?

Yes.

5. To whom are you married?

To MLE, 25 years old, residing at 345 BEC St., Sta. Mesa, Manila,
working as a lawyer.

6. Do you and your wife possess all the civil and legal rights, and have
not been convicted of any crime involving moral turpitude?
 Yes.

7. What are the documents that are available on your end to prove your
capability to adopt?
We both submitted to the courts the following: birth certificate;
marriage certificate; NBI Clearances; Good Moral Character; psychological
evaluation by a competent physician; and income tax returns.

8. Are both of you more than sixteen (16) years older than the age of
the minor that you want to adopt?
Yes. We are both 25. The minor is just 2 years old.

9. Would you be able to support and care for the minor?

Yes. Both of us have stable jobs.

10. How did you know about the minor that you want to adopt?
The Barangay Chairman have informed us about the minor, “Boy
Bata”. We learned that he has no known parents. We also learned through
the same person that the DSWD took custody of the child, and has been taking
care of him since then.
