General General appearance, level of consciousness, orientation Survey Vital Signs - blood pressure - calculate BMI - heart rate, respiratory rate, temperature Skin Skin inspection/general survey Upper - Inspect nails, hands, arms and joints. extremities - Palpate radial pulses on the wrists at the same time. HEENT - Inspect the region of the puncta, conjunctiva and sclera. - Inspect the cornea and lens, using a penlight - inspection of oropharynx and dentition - lymph node palpation Heart - PMI - cardiac auscultation with diaphragm and bell Chest/Lungs - Thorax inspection - Chest percussion posteriorly - chest auscultation anteriorly and posteriorly Abdomen - abdominal inspection - abdominal auscultation in one quadrant - percuss in all quadrants - palpate lightly and deeply in all quadrants Extremities - inspection of extremities and joints - palpate for edema - palpate posterior tibial or dorsalis pedis pulses
