TECHNICAL FOLLOW-UP FORMS

1. Purpose
Customers are requested to fill out the Technical Follow-up Forms for the
transmitter(s) during the guarantee period.

The completed forms should then be sent to THOMSON for analysis and initiation of
any actions to be taken within the scope of the guarantee.

By collecting this technical information, THOMSON is able to monitor the


performance of its transmitters in operation with the aim of providing the best
possible service to its customers.

These actions may be continued once the guarantee period has elapsed in the
framework of a technical assistance agreement.

2. Forms Used
There are four kinds of forms in use:

- RTO: Report of Transmitter System Operation

- TER: Technical Event Report

- MSR: Amplifier Module Statistics Report

- SPR: Spare Parts Request.

2.1 Report of Transmitter System Operation (RTO)


The Report of Transmitter System Operation should mention all significant technical
incidents that have occurred during operation, specifying:
- Description of the event.
- The effect on operation.
This Report of Transmitter System Operation (RTO) should be sent to THOMSON at
regular intervals (to be agreed with the customer).

Whenever a technical incident leads to the failure of a component or sub-assembly,


a Technical Event Report TER should be made out (see § 2.2).

- The Technical Event Report Number of the technical incident should be


recorded in the appropriate column.
Example (TER No 00 001, for the first incident requiring a TER, etc.).

2.2 Technical Event Report (TER)


A Technical Event Report should be drawn up for each technical incident involving
the failure of a component or sub-assembly.

The TER should be sent to THOMSON as soon as possible.

The TER should be numbered chronologically and also recorded in the "TER No"
column of the RTO.

For the first TER, No 00 01, the second TER No 00 02, etc.

All the events indicated in the TER should be described as objectively and clearly as
possible.

Whenever a technical incident involves one or more failed Amplifier Modules, it is


also requested that the customer fill in the MSR form. This will enable a statistical
follow-up to be made on these modules (see § 2.3).

Whenever a technical incident leads to a repair requiring spare parts (components,


modules, cards, etc.) it is necessary to attach to the TER form a spare-part request
form, SPR (see § 2.4).

Note: A copy of the transmitter log book showing the incident should be
attached to each Technical Event Report (TER).

2.3 Amplifier Modules Statistical Report (MSR)


The Amplifier Modules Statistical Report form MSR should be filled in whenever a
technical incident involves one or more failed Amplifier Modules.

The MSR should be sent to THOMSON together with the associated TER.

The MSR should be numbered chronologically and bear the same number as the
associated TER.

For TER, No 00 05, the MSR will be No 00 05, etc.

Full details under each heading of the MSR should be included as follows:

- The number of the Amplifier Block "Block Number": 1, 2 or 3 for the


transmitters comprising several blocks.

- The position "Left or Right" in the Amplifier Block, observing the transmitter
face-on "L/R": L or R.

- The Number (from 1 to 256 ) of the Amplifier Module involved, "Module No".:
184 or 165 etc. (The module number can be found by using the labels inside
the doors of the Amplifier Block).

Attention: indicate one module number only on each line of the form.

- The location code(s) of the faulty component(s), on the module.

"Failed components": F1, Q101 to Q 108, etc.

2.4 Spare Parts Request (SPR)


The Spare Parts Request form should be filled in whenever a technical incident
leads to a repair requiring spare parts (components, modules, cards, etc.)

The SPR should be sent to THOMSON together with the associated TER.

The SPR should be numbered chronologically and bear the same number as the
associated TER.

For TER, No 00 05, the SPR will be No 00 05, etc.

Full details under each heading of the SPR should be included as follows:

- The location code(s) of the faulty component(s), on the module or card.

"Component or module Location": F1, Q101 to CR07, etc.

- The name of the faulty component or module involved.

"Designation": Transistor, Diode, Amplifier Module, etc.

- The Manufacturer's Reference of the faulty component or module.

"Reference": IRFP 460, MUR 120, etc.

- The THOMSON article number of the faulty component or module (shown on


the material or in the user manual).

"THOMSON No": 46,835 or 91,765,800 etc.

- The sub-assembly in which item is used, (indicating as accurately as possible


where the faulty components or modules come from).

"Sub-assembly in which item is used": GESMUL bay, Amplifier


bay, etc.

Note: In the case of difficulty in describing which sub-assembly the failed


elements came from, attach a sketch or photograph showing them in place.

- The amount of parts used for the repairs involved in the TER in question.

"Qty used for repair"

- The amount of parts still available on site after making the repair.

"Qty available on site"

- the amount to be supplied by THOMSON

(To be filled in only when the amount of stock on site has run too low)

- "Qty to be supplied by THOMSON"

- Comments relating to each line may be made, if necessary.

"Observations"

Note: Blank RTO, TER, MSR and SPR forms in the form of WORD* and
