REVIEW COURSE AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

That I, a student of BRAIN TRAIN with business address at Dinorado St. cor. Elon-elon St., Sta. Fe Subd., Batong
Malake, Los Baños, Laguna, hereby acknowledge that I will be receiving the 2020 BRAIN TRAIN UPCAT
REVIEW Manual from Brain Train and will be availing of the BRAIN TRAIN VIRTUAL CLASSROOMS.

That I hereby undertake to hold with utmost confidentiality everything contained in the 2020 BRAIN TRAIN
UPCAT REVIEW manual. hereby undertake not to photocopy or in any way reproduce any part of it or to lend it
to any person or to dispose of it in any manner. I hereby acknowledge that Brain Train has a copyright over this
review manual. It is hereby understood that in case of breach of this undertaking, I shall be liable for whatever
damages, compensatory or otherwise, sustained by Brain Train, which should not be less than two hundred thousand
pesos (P200,000.00). In case the review manual gets lost or stolen, I hereby undertake to submit to Brain Train an
affidavit of its loss within 30 days of the loss. In the absence of any affidavit, the loss of the review manual will be
deemed intentional and I will be penalized accordingly.

That I hereby undertake to hold with utmost confidentiality everything contained in the BRAIN TRAIN
VIRTUAL CLASSROOMS. I hereby undertake not to conduct any video recording, screen shooting, or screen
recording of the BRAIN TRAIN VIRTUAL CLASSROOMS. It is hereby understood that in case of breach of this
undertaking, I shall be automatically dismissed from the classes, and I shall be liable for whatever damages,
compensatory or otherwise, sustained by Brain Train, which should not be less than two hundred thousand pesos
(P200,000.00).

That I hereby bind myself to attend all the review classes prescribed by Brain Train. I agree that I will have to avail
of make-up classes for any meeting I missed (at an additional cost to me), even if I had an excuse for my
absence. It is hereby understood that I am only qualified to take the UPCAT Simulations (examinations) upon
attending all the meetings of the review program.

It is hereby understood that I have received a copy of the Brain Train UPCAT Review Guide. I also understand the
review fee payment scheme including how I can avail of discounts.

That I hereby give consent to BRAIN TRAIN to publish my name and picture in their publicity and marketing
materials if I pass the UPCAT and other College Entrance examinations.
