Data Mapping of Learners for SY 2020-2021

This data mapping activity is crucial for planning for the scheme of teaching for SY
2020-2021. Kindly accomplish diligently so that relevant teaching modality will be decided.

Instructions:
1.   Encode learner to their grade level for SY 2020-2021. Use list of learners during
Early registration for incoming Kinder, Grade 1, Grade 7 and Grade 11. Automatic
enrolment for other grade levels.
2.   There is provided list of options (dropdown list) per cell. No need to type entry fo
columns F to N.
3.   This template shall be accomplished by the adviser or any teacher who has
access to information required in the mapping tool. No sectioning yet per grade
level.

4.   The school head shall see to it that the template is accurately accomplished.

5.   Accomplished template per grade level shall be consolidated and submitted by
the School ICT Coordinator to the District Planning Officer Designate on or before
May 15, 2020.
6.   For secondary schools (JHS and SHS), the school consolidated report shall be
submitted to the Municipal ICT Coordinator on or before May 15, 2020.
7.   The District and Municipal ICT Coordinator shall submit the complete
consolidated report to the Division on or before May 20, 2020.
8. For easy consolidation of report, do not alter the template.
9. Maximize electronic means of communication (text, messenger, etc), and other
strategies while strictly observing social distancing, wearing of masks and other
precautionary measures
