Instructions: Interviews are an effective means of obtaining feedback from trainees, supervisors and other
respondents. Interviews target a baseline of tasks or objectives, using questions that are clear and
unambiguous. Ensure to plan open-ended questions because of the results received via this method.
Carefully fill out the following document to capture the groups or individuals who will be interviewed and the
questions that will be asked during the interview process.

Introduction to the Interviewee(s): The purpose of this interview is to collect your feedback on how to
improve Insert Name training to better prepare you to do your jobs. We are trying to determine if the training
you received, prepared you to do your job effectively and to determine if it was fit for purpose. Points from this
interview will be analysed and from that, findings and recommendations will be constructed and reported to
the Training Authority (TA). The following questions will be asked based on the Specification/ Specialty
Specification Task List that you were provided.
