VALUES, HABITS, PRACTICES ON HEALTH PROMOTION,

MAINTENANCE AND DISEASE PREVENTION
Mr. and Ms. Planet had immunization of BCG and TTs up to second dose. Her
children received complete immunization such as BCG, Hepa B, DPT, OPV HiB,
Rotavirus, PCV, Influenza, Measles, MMR, Varicella and Hepa A. The family uses
some herbal plants for medication such as oregano for cough, kutsay for bruise, and
boiled guava leaves for cleansing wounds when they don't have enough money to buy
over the counter medications. They also use medicines like Paracetamol for fever,
Kremel S, Loperamide, Decolgen for fever, body pains, head ache and tooth aches,
upset stomach, diarrhea, and colds respectively.
According to Mrs. Planet, all members of the family have enough rest and
sleep. She stated that there is no proper routine for their exercise and the only exercise
that she can point out is walking, and it’s only happens when she walked her daughter
to school and fetches her after school. Watching television and listening to radio
serves as relaxation to her family. She also feels relaxed when she’s watching her
children playing and teaching and helping them with their school assignments. We
observed that despite hardship in life most especially that she has a life-threatening
disease; Mrs. Planet try her best to become a good mother and provider to her kids.
Every day she wakes up early to prepare their foods and do household chores. She
endures the pain that she has in order to do her obligation as an “Ilaw ng Tahanan”
and struggling to overcome this pain for their children to achieved higher education.
Her three daughters also strive hard to finish their studies even though their allowance
is not enough.

FIRST LEVEL OF ASSESSMENT
I.PRESENCE OF WELLNESS CONDITION
These are nursing judgment on wellness state condition based on a Family’s
Performance, current competencies is clinical data but no explicit expression of
client’s desire.
Readiness for enhance wellness state condition based on clients current
competence, clinical data and explicit expression of desire to achieved a higher level
of state or function in a specific area on health promotion and maintenance.
A. POTENTIAL FOR ENHANCED CAPABILITY FOR:
1. HEALTHY LIFESTYLE
The Planet family is fonds in eating vegetables like camote tops, malunggay,
munggo, squash, upo, beans and other leafy vegetables. Every member of the
family is fit and none of them is undernourished or ever nourished.

2. Health maintenance/Health Management

The Planet family uses herbal medicines for common illnesses as well as drug
medicines and they are still consulting the RHU physician if symptoms get severe.

Ms. Planet claimed that her family is Roman Catholic but they don’t go to church
regularly because of time and financial constraint but she further stated that she
always remembers to thank God for all the blessings and asks His guidance every day.

 The Planet family loves to eat fruits and vegetables and they are very
vocal to share their desire to plant vegetables if only they have garden or
space to plant.
2. Health Maintenance/Health Management
Health education will be discussed to Planet family and will be
taught of specific competencies to ensure that their home is good for
maintaining health and personal development through modification,
manipulation or management to eliminate health threats or risks.
3. Parenting
Ms. Planet is willing to improve family communication, role
assumptions, relationship and interaction.
4. Spiritual well-being
Through proper counseling Ms. Planet will be encouraged to find
time going to church regularly with the whole family.
II. PRESENCE OF HEALTH TREATS
There are conditions that are conducted that are conductive to disease and
accident, or may result to failure to maintain wellness or realize health potential.

III. PRESENCE OF HEALTH DEFICITS

Last April, 2019 Ms. Planet was diagnosed with a breast cancer which give
him a condition of health breakdown. Her family doesn’t have any manifestation of
diseases except for common cough and colds.
