PROJECT STRATEGY
Describe your project strategy and plan in detail, and demonstrate how the proposed project responds to this PFA. Do not exceed 10 pages. You may delete this
instructional text box.

A. Background
 Clearly identify and describe the research finding you aim to disseminate, including supporting statistical
tables and figures as needed.
 Identify the evidence gap being filled by the described research finding.
o What patient-centered care problem(s) does the research finding or product address? Does it close
or narrow an evidence gap that is important to patients and stakeholders? How?
o Describe how the evidence gap has evolved since the funding of the original PCORI research award.
Does the gap persist?
o How does this finding fit within the existing body of evidence? Explain how the current finding
supports, augments, or differs from related evidence.

B. Significance
 What health or health care decision will your finding inform? Who are the primary decision makers and in
what context does this decision take place?
 How will the dissemination and implementation (D&I) of the proposed research finding lead to improved
health care and outcomes?
 Describe how the research finding is useful to end-users.
o Why would they want to use the finding?
o What is the anticipated impact of your D&I effort on patient and provider decision making, clinical
practice, and policy making?
 Provide a rationale regarding the readiness of this finding for D&I.
o Describe the strength of the results being proposed for dissemination, including their clinical and
statistical significance and meaningfulness.
o Describe the generalizability of the results being proposed for dissemination to populations beyond
the study sample.
o Describe the anticipated readiness of current systems (people, institutions, groups) to use the results
of this project if successful.

C. Approach and Evaluation


 Describe the proposed approach for disseminating the research findings.
 State the D&I plan’s specific aims and objectives.

 Describe dissemination settings and end-users, including how they will be selected and their
representativeness. Be as specific as possible. Specify healthcare settings as well as types of patients,
caregivers, healthcare providers, and other key stakeholders that will be targeted in your D&I approach.

 Describe the projected reach of this D&I initiative in terms of:


o Absolute number: Total number of individuals you expect to reach with this D&I initiative
o Proportion: Expected reach of this D&I initiative relative to the broader population of individuals who
stand to benefit from the results being disseminated.
o Representativeness: Consideration of how those being targeted with this D&I initiative are similar or
different from the broader population of individuals who stand to benefit from the results being
disseminated.
 What factors may act as barriers to successfully disseminating and implementing the research finding
within the context of the proposed project, and how might you mitigate them?
 Describe the D&I framework or model you will use to anchor and inform the project design, outcomes,
and evaluation plan.
 Specify the measurable process, short-term, intermediate, and longer-term outcomes you will use to
capture timely information on the effectiveness of the proposed D&I activities, including impact on the
end user. How will you know if you have been successful?
 Describe the potential for sustainability of this D&I effort and the potential for scalability to other
audiences and settings, including a clear path for future efforts to bring these research results into wider
use. If this D&I effort is successful, what stakeholders would be interested in implementing it more
broadly and why? What would be the advantages or obstacles to their continuing this type of effort?

D. Engagement Plan
 Outline how patients and other stakeholders will participate as partners in various phases of the proposed
project.
 Clearly identify individuals, organizations, and informal networks you will partner with to disseminate the
research findings to the end-users. Note their importance to reaching particular end-users.
o Will you be partnering with the same individuals, organizations, and informal networks you
collaborated with during the original PCORI project? Will you be adding new partners for this D&I
effort? Describe your rationale in either/both cases.
o How does the research finding or product fit with and advance the mission and goals of these
partners?
o What factors might help or hinder their willingness to work with you?
 Clearly describe the willingness and readiness of the healthcare systems and settings in which
dissemination will occur to use and embrace these research results.
 Describe how this D&I plan reflects the PCORI engagement principles of reciprocal relationships, co-
learning, partnerships, transparency, honesty, and trust.

E. Dissemination Team and Environment


 Describe the multidisciplinary team, including appropriate stakeholders and experts to help shape messages
to target audiences, provide context for the evidence for adopters, and assess the effects of dissemination
and implementation activities.
 Describe the dissemination team’s capability to accomplish the goals of the proposed dissemination project
and the appropriateness of the organizational structure and described resources.
 Describe the facilities and resources (e.g., collaborative or partnering arrangements) that will be used to
carry out the proposed project
 Demonstrate that there is sufficient institutional support for the proposed project to be successful

For additional guidance, refer to Section 5, titled “Human Subjects Research Policy,” from the Supplemental Grant Application Instructions for All Competing
Applications and Progress Reports, which was issued by the U.S. Department of Health and Human Services. Also refer to the Required Education of Key Personnel on
the Protection of Human Subject Participants requirement as you complete this section. For detailed instructions, review the Application Guidelines for the PFA. Do
not exceed five pages.

Describe the protection of human subjects who will be involved in your project, as applicable.
