2. Students are entitled to one free copy of transcript once graduated. A fee of RM 10.00 is charged for each
additional copy of transcript.

3. Student may allow another person to collect the transcript on his/her behalf with a valid authorization letter.
(Please submit the authorization letter together with proxy form).

4. Student must make sure that all information is correct upon submitting this form to Graduate School of
Management.
Graduate School of Management will not in any way print a new transcript due to misinformation.
Please fill in the personal info form at the back (Compulsory).

5. Student must present the receipt of payment while collecting the transcript.
