SERVICE AGREEMENT

This Agreement is entered into by and between:

PHILIPPINE PORTS AUTHORITY – PORT MANAGEMENT OFFICE OF

BATANGAS, with official address at Port Access Road, Calicanto, Batangas City, Batangas,
represented by JOSELITO O. SINOCRUZ PhD, Port Manager, PMO Batangas, herein referred to
as “FIRST PARTY”,

and

ELEUTERIO A. ANDAL, JR., of legal age, Filipino, Trainor/Conductor, herein referred

to as “SECOND PARTY”

WITNESSETH AND AGREED AS FOLLOWS:

1. The First Party is in need of the service of the Second Party, to serve as Trainor/Conductor
for PMO chorale members, for the Intra-Cluster pre-pageant for the SiReyna ng Pantalan
and the Chorale Singing Contest on November 26, 2019;

2. The Second Party has signified his intention, to which the First Party has Accepted, to
serve as Trainor/Conductor for PMO chorale members;

3. In consideration of the full, satisfactory and faithful performance of the Second Party of all
its obligations under this Agreement, First Party hereby agrees to pay the Second Party the
amount of Fifteen Thousand Pesos (P15,000.00);

4. That the Second Party shall provide lesson/practice to the PMO chorale members from
November 6 to 26, 2019;

5. That the Second Party shall serve as Conductor of the PMO chorale on November 26, 2019;

6. That it is understood that this agreement does not create an employer and employee
relationship between the First party and Second Party.

IN WITNESS WHEREOF, the parties hereunto signed this Agreement this 4 th day of
November 2019, at Batangas City, Batangas, Philippines.
