REGISTRATION OF BIRTH, MARRIAGE CERTIFICATES


AND DEATH CERTIFICATES
ABOUT THE SERVICE

Republic Act 3753 mandates the establishment of a civil register in the Philippines
where acts, events, legal instruments and court decrees concerning the civil status
of person shall be recorded.

The birth of the child, being a vital event of a person, shall be registered within
thirty (30) days from the time of birth in the office of the Civil Registrar of the
municipality where the birth occurred.

For ordinary marriage, the time for submission of the Certificate of marriage is
fifteen (15) days following the solemnization of marriage while for marriage exempt
from license requirement, the prescribed period is thirty (30) days, at the place
where the marriage was solemnized.

While it shall be the responsibility of the nearest relative or spouse who has
knowledge of the death to report the same within forty-eight (48) hours if the
deceased died without medical attendance.

The Health Officer shall examine and sign the death certificate and direct the
registration of the death certificate to the Office of the Civil Registrar within the
reglementary period of thirty (30) days.

APPLYING FOR MARRIAGE LICENSE


ABOUT THE SERVICE

Where a marriage license is required, each of the contracting parties shall file
separate sworn application for such license with the proper local civil registrar of
the place where either or both of the contracting parties reside.

The local civil registrar concerned shall enter all applications for marriage files with
him in a registry book strictly in the order in which the same are received.

When the license is issued, the same shall be valid in any part of the Philippines
for a period of one hundred twenty (120) days from the date of issue, and shall be
deemed automatically cancelled at the expiration of said period if the contracting
parties have not made use of it.

respective embassy in the Philippines or in the absence of an embassy in the

Delayed registration of birth, like ordinary registration made at the time of the
event, shall be filed at the Office of the Civil Registrar of the place where the event
occurred following the lapse of thirty (30) days.

ABOUT THE SERVICE

These services are available as per requst of the MHO and request from other
clinics/hospitals.

REQUIREMENT(S)

For blood chemistry examinations, advise patient to observe NPO (Nothing per
Orem or “Mouth”) or fasting prior to testing.
For routine microscopy, instruct patient for proper specimen collection.
For water analysis, instruct client for proper procedures of water sampling.

SCHEDULE OF EXAMINATIONS

 For blood chemistry, schedule is 7:00 to 8:30 a.m. every Wednesday


 For routine microscopy, schedule of receiving of specimen is 8:00 a.m. to 10:00
a.m. and 1:00 p.m. to 4:00 p.m. daily. Specimens coming beyond this time are
advised to come back the next day.
 Prenatal laboratory requests are accommodated on Fridays, 8:00 to10:00 a.m.
and 1:00 to 4:00 p.m. (MHO Main) and Wednesdays from 8:00 a.m. to 10:00
am. (MHO Annex)
 Sanitary Permits laboratory requirements are accommodated every Mondays,
Tuesdays, Thursdays and Fridays from 8:00 a.m. to 10:00 a.m.

The Municipal Health Office (MHO) is a Newborn Screening Facility in compliance


with Republic Act 9288, Newborn Screening Act of 2004. A public health program
at early identification of infants who are affceted by certain
genetic/metabolic/infectious conditions which could lead to significant reduction of
morbidity, mortality, and associted disabilities in affacetd infants.

Newborn age 24 hours to 72 hours may avail of the service.

FILING OF SWORN STATEMENT


ABOUT THE SERVICE

Section 202 of Republic Act 7160. Any person, natural or juridical, owning or
administering real property or his authorized representative is required to file a
sworn statement declaring the true current and fair market value of his property,
whether previously declared or undeclared, taxable or exempt, with the provincial,
city or municipal assessor where the property is located once every three (3) years
during the period from January first (1 st) to June thirtieth (30th) commencing with
calendar year 1992.

Section 203 of Republic Act 7160. Duty of Person Acquiring Real Property or
Making Improvement Thereon. Any person, who acquires any kind of real property
or his authorized representative, shall file a sworn statement declaring the true
current and fair market value of the subject property with theaAssessor where the
property is located within sixty (60) days from date of execution of the deed of sale
or conveyance.

ABOUT THE SERVICE

Republic Act 7610 mandates the DSWD to issue a travel clearance to


unaccompanied minors traveling abroad or a written permit or justification from the
child’s parents or legal guardians (Article IV, Section 8a) as a measure to protect
children and prevent trafficking.

ABOUT THE SERVICE

The Municipal Tourism Office issues a Certificate of Endorsement for individual


Mayor’s/Business Permit requiring all tourism frontliners to undergo a mandatory
seminar and training on tourism.

● Accomplished Municipal Tourism Frontliner’s Form / Profile
(Municipal Ordinance No. 150, Series of 2002)
● Municipal Tourism Frontliner’s Enhancement Program Certificate
(Executive Order No. 011, Series of 2012)

2. For Individual / Establishments / Travel & Tour Operations:


● Certification of Employment
● Accomplished Municipal Tourism Frontliner’s Form / Profile

3. For Foreign Tour Guides:
