Pearl GTL Safety Alert – 020

CONCRETE TRUCK ACCESS ROUTES
Incident Location:
Pearl QSGTL – Construction Sites
Date of Incident:
19 September 2007
Description :
Very often concrete delivery truck drivers
and people working in excavations near
roads / paths are exposed to very high risk
when the trucks are to be maneuvered into
areas with very narrow roads and pathways
for direct delivery of concrete. The drivers
are expected to reverse in / out of these
areas and with very little room for error or
misjudgment may end up falling into
excavations. This could have very serious People and equipment in excavations?
implications and should be avoided as far Very narrow access routes?
as possible.
Alternative area – pouring by means of extension chute from safe work
area and distance away from excavation?
Initial Findings / Learning’s:
• Safe access and egress routes not wide enough for safe delivery of concrete – Very small distances
between truck wheels and excavation edges. In some cases less than 500mm
• People / equipment inside adjacent excavations not being removed when trucks are moving in and out of
areas with space constraints.
• In many cases the excavation side walls are not battered or shored – due to space constraints, design lay
outs, and may not be able to handle the heavy weight of the fully loaded trucks.
• All of the abovementioned also applicable to the use of other construction equipment such as Bobcats,
Small Excavators, Rollers, Back hoes, and Pick ups required to operate and move on embankments or
between concrete plinths and structures.
“Rather take a little longer to do the pour – than putting your people’s lives at risk”
Immediate / remedial actions to be taken to prevent loss :
• Where feasible these trucks should come into the area – discharge the concrete and leave without having to
reverse the truck in areas with space constraints.
• Supervision to ensure that all people in close vicinity of moving truck are removed from work areas such as
excavations around plinths, before allowing vehicles to enter these areas.
• Trained – Competent banks men must be appointed and placed at these construction activities.
• TSTI’s and Risk Analysis to be reviewed and include these high risk activities.
• Alternative methods to be considered to pour concrete – Eg: By means of extension chutes from a safe distance,
with wheelbarrows, pumping or the use of other construction equipment from truck to final place of concrete.
