Dear Ms. Tavera:

I formally endorse Mr. Johncary Angelo Laysico, an undergraduate psychology student of De La Salle
University. Mr. Laysico expressed intention to do the required 200 hour practicum in your institution. For this
term, the practicum calendar starts on September 10, 2018 and ends on December 8, 2018 Students
enrolled in practicum are expected to be able to administer psychological tests, profile test results, plan
basic group process exercises, assist in facilitating group process exercises, observe group process,
conduct individual and group interviews, do basic research, and, gather and analyze quantitative and
qualitative data.
If accepted, the student shall be assigned to an on-site supervisor. Fifty percent of the student’s grade in
practicum will be based on the supervisor’s rating.

TO THE DLSU PSYCHOLOGY PRACTICUM STUDENT’S ON-SITE SUPERVISOR:

Outlined below are the expectations of the Psychology Department regarding your role as the On-Site
Supervisor of our intern/s. Please feel free to contact us for any clarification. Our telephone number is
524-4611 loc. 560. Thank you in advance for the cooperation and help you will be extending to us in the
course of training our psychology major/s.

1. The On-Site Supervisor, before accepting the Intern, ascertains whether the student has the
skills/capabilities to do the work required of him/her in the department or office. Rejection or
acceptance of the trainee is the On-Site Supervisor's prerogative.

2. The On-Site Supervisor, or his/her duly-designated representative, signs the intern’s Daily Time
Record Form to certify that the student had been working for the specified number of hours for
that day.

3. The On-Site Supervisor assigns tasks/responsibilities to the intern so that the student gains a
better understanding and appreciation of the chosen field of internship. Following up and guiding
the intern in the fulfillment of such tasks are among the responsibilities of the On-Site Supervisor.
At the start of the student's training with the organization, a Job Description Form/Internship Plan
is to be accomplished by the On-Site Supervisor, and the tasks detailed on this form are to be
referred back to in assessing the quality and quantity of the practicum student's work at the end
of the term.


4. The On-Site Supervisor assists the student in preparing documents required by CHED Memo No.
104 s. 2017 such as (a) Institutional Memorandum of Agreement, (b) Memorandum of Agreement
with the Student, and (c) Internship Plan annexed to the latter.

5. At the end of the term, the On-Site Supervisor completes the Department's Performance Appraisal
Form and forwards it in a sealed envelope (signed across the seal) to the Faculty Adviser.

6. At the end of the term, the On-Site Supervisor is expected to provide the intern with a Certificate of
Completion of Practicum Work typewritten on the organization's letterhead, to attest that the student has
fulfilled the required number of hours of work.
