AFFIDAVIT OF LOSS

That I, MARIA FE G. SARMIENTO, of legal age, married to Gregory A.

Sarmiento, Filipino, resident of Poblacion, San Manuel, Tarlac, after having been
dully sworn to in accordance with law, do hereby depose and state - THAT:
1. I am the buyer of a parcel of land by virtue of Deed of Absolute Sale is
hereto attached for reference;

2. The subject property, which is covered by Transfer Certificate of Title

(TCT) No. 297406, of the Registry of Deeds of the Province of Tarlac, still
in the name of the former owner Francisco Daligdig, is more particularly
described as follows:
TRANSFER CERTIFICATE TITLE NO. 297406

“A parcel of land (Lot 184, of the subdivision plan (LRC) Psd-

154282, being a portion of Lot 2-A-2. Psd-57259) situated in the
Barangay of Culubot, Municipality of San Manuel, Province of
Tarlac, xxx containing an area of TWENTY THOUSAND SIX
HUNDRED SEVENTY (20,670) square meters, more or less.”

3. Upon the execution of the said Deed of Absolute Sale, Francisco Daligdig
turned over to me the original Owner’s Copy of TCT No. 297406;

4. I then placed the said original Owner’s Copy of TCT No. 297406 inside a
long blue expanding envelope which I in turn safe kept in one of the drawer
of a wooden cabinet at our residence in Barangay Poblacion, San Manuel,
Tarlac;

5. Recently, I decided to process the registration and titling of the Subject

Property in my name which would require, among others, the presentation of
the original Owner’s Copy of TCT No. 297406 to the Registry of Deeds of
the Province of Tarlac;

6. However, when I looked for the original Owner’s Copy of TCT No. 297406
in the wooden cabinet where I remember keeping it, I could no longer find
the same;

7. I even scoured every corner of our house hoping that it was just misplaced
somewhere but all efforts made proved futile;
 8. With the foregoing, I have reason to believe that the original owner’s copy
of TCT No. 297406 is now lost and beyond recovery.

9. That I am executing this affidavit to inform the proper authorities concerned

about the loss of said original Owner’s Copy of TCT No. 297406 for
possible replacement in lieu of the lost one and for whatever legal intents
and purposes it may serve.
