Answer any two Questions. Each question carries 10 marks Marks Level
Outcome
The government wants to get the required information from people about
their symptoms to check whether the people are infected by corona virus
1 a or not. Design the form to get the above information using tags such as 5 CO3 K3
input tag, select tab, label tag, dropdown list, checkbox, radio and submit
button.
Write a PHP file for login process. This file should contain
 Make a connection from MySQL database to login form
 Create a login table under user database with the required fields

 USERNAME field should be designated as UNIQUE.
 If any of these operations cause an error, stop execution and print
the error message.
The University wants to create a dynamic page holding an event or
conference. People who are interested in attending the conference create a
register button for them. By clicking on register button, using onload
event a form have to load for registration with following details such as
Team name, Department, College Name (select from dropdown list),

payment details (bank information), Abstract Submission (Text Area),
Phone Number, Email id, Address for communication, terms and
conditions and place submit button at the end of the form. By Clicking on
Submit button, details should be submitted and display the user
information with message” successfully registered”.
b Outline the structure of SOAP Request and Response message with suitable 5 CO5 K2

3 a Create an xml file with name student.xml to display student academic 5 CO4 K3
information. Design the scheme for the above xml file and also add XSD
