1. Reference your application for grant of Short Service Commission in the Armed Forces Medical Services (AFMS).
2. You are requested to report for interview on 07 Mar 2018 at 0730 hrs at INHS Asvini,Colaba, Mumbai-400005. On
selection you are required to undergo a Medical Examination at INHS Asvini,Colaba, Mumbai-400005, which may take two days.
3. You are also requested to bring the following certificates in original alongwith three attested copies of each certificate on the
date of interview without fail:-
(a) Matriculation Certificate (Class X) / Birth Certificate issued by Municipal Corporation as a proof of name and date of
birth.
(b) Aadhar Card.
(c) MBBS Degree Certificate, PG Degree Certificate.
(d) MD/MS/DNB/Diploma Pass Certificate & Degree from the university (if applicable).
(e) Permanent Medical Registration Certificate.
(f) Final MBBS attempt cum Transcript Certificate (including final professional MBBS Pt-I & Pt-II)
(g) Internship Completion Certificate.
(h) Ten passport size recent photographs of self, Identity & Residence proof.
(j) NOC from employer, if employed.
(k) NCC Training Certificate, if any.
(l) Printout of the application form submitted online.
(m) Attestation Form in triplicate duly filled in and attested by Competent Authority(Refer Para 4 below).
(n) National Board of Examination Certificate for Foreign Medical Graduates.
(o) Passport for Foreign Medical Graduates.
(p) Gazatte Notification or any other authority clearly highlighting your new name, if there is any change in the name after
Matriculation (10th Class).
4. The Attestation Form is available on the website. Candidates are required to bring the completed Police Verification
Report (PVR) Form ( in triplicate) at the time of interview, positively duly filled in and attested by the authority as mentioned
at Page No. 07 of Attestation Form.
5. TA/DA for onward & return journey in connection with interview is payable after performance of journey only to those
candidates who are appearing for the first time under the existing TA/DA rules. The candidates who have already appeared for AMC
SSC Selection Board interview and have claimed TA/DA once are not entitled to further TA/DA. TA/DA is not admissible to the
candidates residing in the Municipal limits of Mumbai.
6. You will have to make your own arrangement for accommodation and transport for the duration of your stay in Mumbai.
7. Please read all the instructions carefully for strict compliance as given at Appendix.

INSTRUCTIONS FOR COMPLIANCE BY THE CANDIDATES
1. Bring this call letter along with five copies of your recent passport size photographs, original and three attested
copies of all the certificates and documents without fail while coming for interview.
2. Be available for interview in your turn.
3. Maintain silence and discipline while waiting for interview within the Hospital.
4. Switch off your Mobile Phone during the selection process.
5. Result will be announced on the same day after interview. Successful candidates will be required to submit
photographs, attested copies of certificates/documents and get the same verified with the original certificates.
Necessary guidance regarding verification of certificates/documents, medical examination and Attestation Form etc
will be given by the concerned staff of the Office of DGAFMS/DG-1A.
6. The Attestation Form for Police Verification is available on the website. Applicants are required to download
the Police Verification/Attestation Form and fill in the same in three(03) copies and deposit it to the staff at interview
board. In case of his/her selection, police verification of character antecedents of the candidate will be processed by
this office through the concerned District Magistrate Office.
7. Obtain TA/DA forms if TA/DA is to be claimed. Read and complete the TA/DA forms carefully.
8. The candidates claming TA/DA should fill up TA/DA forms correctly and carefully. TA/DA claim is to be
submitted duly completed and signed alongwith journey tickets in original or attested photocopies thereof, if original
tickets are not in possession. Bring a photocopy of self account cheque or a cancelled cheque issued from bank to be
enclosed with TA/DA forms and IFS code of Bank branch.
9. Giving incomplete/wrong information and not signing and attaching journey tickets may result in delay or
non-admittance of your TA/DA claim by the concerned authority for which this office will not be responsible in any
manner.
10. After verification of certificates/documents, successful candidiates will be medically examined on the same
day or next working day of interview at which may take two days.
11. The candidates should not absent themselves at any stage during the process of their selection/medical
examination.
12. The successful candidates will be required to fill up page 01 of prescribed proforma for Medical Examination
on Entry AFMSF-2 (Ver 2002) and a 'CERTIFICATE' regarding their status before the medical examination. Giving
incomplete, incorrect, misleading/false information therein will be viewed seriously for which they will be liable for
initiation of disciplinary action or termination of service if granted commission in the AMC as Short Service
Commissioned officer. Please note that candidates found medically 'UNFIT' will be explained the procedure for
'Appeal Medical Board' which is to be conducted within 42 days from the first day of your medical examination at
desired Service hospital.
