First Step:
1. Letter of Intent from the Mayor (refer item 14, paragraph a of the memo)
- Include specific intended purpose or use of school
- Specify number of classrooms per school, time frame of use (start to end)
2. Certification that LGU duly assessed its inadequacy of facilities, & recommend school
for its use (refer item 14, paragraphs b & c of the memo)
3. Certification by Local Health Officer that facilities within school is suitable for specific
intended purpose (refer item 14, paragraph d of the memo)
4. Planned Management of the facility (refer DOH Memo No. 123, 2020)
Second Step:
Third Step:
9. When approved by RO, School Head will notify LGU (refer template in the memo)
10. LGU will make TAC duly signed by Mayor & School Head and notarized (refer
template in the memo)
11. Mayor and School Head will sign Inventory of Facilities (refer Annex A of the
memo)

2. Office job orders will have separate work scheme following 4-Day Work Week
arrangement schedule.
4. See matrix below for the on-site skeleton staffing schedule effective today until May
15, 2020 or upon the duration of the GCQ. A separate weekly matrix for security guards
will be provided to make sure school’s safety will be maintained.
5. All NTP are hereby expected to accomplish tasks appertaining respective KRAs even
Working Home-Based pursuant to DM No. 43, s. 2020. This manifests that you are
directed further to submit reports on time required of you.
6. Moreover, should the LGU commences the utilization of school as an isolation area
for COVID19 casualties (PUIs/PUMs), all skeleton staffs will no longer report.
