CONCEPTION CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This Contract is made and executed by and between:

__________________, Filipino, of legal age, single, with residence and

post-office address at Block 5, Lot 10, La Terraza PhaseB, Bucandandala 1,
Imus City, Cavite, hereinafter referred to as the FIRST PARTY;

-and-

___________________, of legal age, single, with residence and post-

office address at Block 5, Lot 10, La Terraza PhaseB, Bucandandala 1, Imus
City, Cavite, hereinafter referred to as the SECOND PARTY;

WITNESSETH, THAT:

WHEREAS, the FIRST PARTY is the recipient of sperm specimen;

WHEREAS, the SECOND PARTY is the donor of sperm specimen;

NOW THEREFORE, for and in consideration of the foregoing premises,

the parties have agreed on the following:

1. The parties abide to pursue and carry out CONTRACTUAL

CONCEPTION as a means of mutual procreation considering that they
are now living with each other as husband and wife, and without
capacity to beget a child unless it is done through artificial insemination;

2. The FIRST PARTY shall receive spermatozoa taken from the SECOND
PARTY through a medical procedure called “In Vitro Fertilization” (IVF);

3. The period of this Contract shall commence to run from the signing of
the parties and the same shall end upon successful conception and
delivery of the child;

4. The SECOND PARTY shall assume the obligations of a biological

father upon delivery of the child subject to the provisions of Articles 40
and 41 of the New Civil Code of the Philippines;

5. The obligations of the SECOND PARTY shall include, but not limited to,
acknowledgment of the child as an illegitimate child should there be no
marriage celebrated between the parties prior to his/her birth, child’s
use of the father’s surname, child’s entitlement to support and parental
authority over the child;

6. That the parties hereto bind themselves to faithfully comply with the
terms and conditions of this Contract.
 IN WITNESS WHEREOF, we have signed these presents this 22 nd day of
September 2016 at City of Cabuyao, Laguna.
