ANSAP Advisory: IVT Card

Intravenous Therapy nurses whose IVT licenses are about to expire or


have lost their IVT cards, the Association of Nursing Service Administrators
of the Philippines (ANSAP) reiterates the following procedures for renewal,
revalidation or loss of IV Therapy Card.

Renewal/Revalidation of IV Therapy Card

The IV Therapy Card is renewable every three (3) years for all IV Therapy
Nurses, Preceptors and Trainers. Renewal shall require completion of IV Related
Updates equivalent to 24 CEU or Continuing Education Units as approved by
ANSAP.

Requirements for Renewal of IV Therapy Card

Please compile all requirements, including processing fee, in one envelope.


Incomplete requirements will NOT be processed.

1. Current PRC ID Card - Photocopy


2. IVT Card - Original
3. IVT Update Certificates of Attendance - equivalent to 24 CEU - Original and
Photocopy
4. Two (2) pieces of 2x2 ID picture (light green background)
5. Processing Fee of Php 400.00
6. For change in status, attach a photocopy of Marriage Contract.

Loss of IV Therapy Card

Replacement of lost IV Therapy Card may be obtained from the National ANSAP
office upon submission of the following:

1. Affidavit of Loss
2. Certificate of Attendance, Basic IVT Training - Original and Photocopy
3. Certificate of Training Issued by ANSAP - Original and Photocopy
4. Two (2) pieces of 2x2 ID picture (light green background)
5. Processing Fee of Php 400.00

To reach the ANSAP office:

From Manila, take a jeepney, bus or FX taxi bound for Fairview and get off right at
the Lung Center of the Philippines.

From southbound EDSA (Cubao, Makati), take a Northbound bus (i.e. Malanday,
MCU Caloocan, SM North Edsa, Trinoma) or take the MRT and get off at Quezon
Avenue station. From there, take an FX taxi or jeepney bound for Fairview and get off
right at the Lung Center of the Philippines.

From northbound EDSA (Caloocan, Valenzuela), take a Southbound bus (i.e.


Cubao, EDSA Crossing, Buendia, Pasay) and get off at Quezon Avenue. From there,
take an FX taxi or jeepney bound for Fairview and get off right at the Lung Center of
the Philippines.
