AFFIDAVIT OF LOSS

I, ERNEST GABRIEL REYES, Filipino, of legal age, single and a resident of

Esperat _____________, Zamboanga City after having been duly sworn in
accordance with law, hereby depose and say that:

1. I am a holder of a ATM card issued by MetroBank in Zamboanga City;

2. On April 16, 2020, I went to the grocery store;

3. When I arrived home, I noticed that the said card was missing, I
probably misplaced or dropped it;

4. I exerted all and diligent extra efforts to find and locate the said card
and in all places which I might have placed or kept it but the same
could no longer be located;

5. For all intents and purposes, I am now declaring the said card cancelled
and lost and hereby undertake that the card was not in the possession
to any third person.

6. I execute this affidavit to attest to the truth of the foregoing facts based
on personal knowledge and as part of the requirements for the issuance
of a new one in lieu of the lost one.
