AFFIDAVIT OF ONENESS OF PERSONALITY

I, MICHAEL OBLIFIAS, of legal age, Filipino, married and with

address at Purok 3, Tablon, Cagayan de Oro City, Misamis Oriental,
Philippines, after having been duly sworn to in accordance with law do
hereby depose and say:

1. That my name as registered in my Certificate of Live Birth is

MICHAEL OBLIFIAS;

2. That in my Certificate of Live Birth, my mother’s name is reflected

as ENCARNACION RAMOS;

3. That the entries in my Certificate of Marriage did not follow the

entries of my Certificate of Live Birth;

4. That in my Certificate of Marriage, my name is reflected as

MICHAEL SALUDES OBLIFIAS when in fact my registered name
is MICHAEL OBLIFIAS only;

5. That the name of my mother as reflected in my Certificate of

Marriage is GLORIA SALES SALUDES when in fact in my
Certificate of Live Birth it is reflected as ENCARNACION
RAMOS;

6. That MICHAEL OBLIFIAS, as registered with the Philippine

Statistics Authority, is the same MICHAEL SALUDES OBLIFIAS
who was married to Gwendolyn Lim Abasola on October 5, 2012,
at Holy Rosary Parish Church, Zone 2, Agusan, Cagayan de Oro
City, Mis. Or. as reflected in the Certificate of Marriage issued by
the Philippine Statistics Authority;

7. That the discrepancy between my Certificate of Live Birth and my

Certificate of Marriage is due only to a clerical error;

8. That I execute this affidavit attesting to the foregoing facts for the
purpose of securing a Philippine Passport in my name, MICHAEL
OBLIFIAS as reflected in my Certificate of Live Birth and for
whatever legal purposes it may serve.
 IN WITNESS WHEREOF, I hereunto affix my signature this 8th day
of December 2015 at Cagayan de Oro City, Philippines.
