STAFF PERFORMANCE ASSESSMENT (SPA) REPORT
This SPA report should be read and completed in conjunction with the supervisor. Please note that the
content of this performance appraisal can be used for salary increment, promotions and as a
reference for future employment. Input in the SPA required from the staff and the Supervisor.

PART 1: STAFF SELF ASSESSMENT (To be completed by the Staff)


Briefly describe the main responsibilities and duties you undertook during the reporting period and
what you consider your major achievement/s in line with your expected output and the work plan, as
agreed between you and your supervisor (maximum 2000 characters)

My main responsibilities and duties are to print CPI weekly, monthly, quarterly and semi-annual forms
for all regions Freetown, Bo, Kenema, Makeni and Kono. Collection of the previous electronics monthly
data entry worksheet from section head and completed verified data collection forms from Statistician
and distributes to others data entry officers and CPI monthly data entry. After all has finish entry
collected and hand over to the Statisticians. And any other jobs assigned to me.
