Describe the instructional activities that will occur PRIOR to the SRT activity and how you will introduce
the SRT activity.
This tool will be used as an introduction to the topic, so I would give a brief intro to what we would be doing
today and then we would begin the topic.
Describe the purpose of the SRT activity (check all that apply):
☒ Assess prior knowledge ☒ Anticipatory set (Create interest in a topic) ☒ To illuminate common
misconceptions ☐ Formative assessment of content knowledge (for purpose of differentiation and
mastery for ALL students) ☐ Summative assessment of content knowledge ☐ Test preparation
☐ Survey/Poll ☒ Discussion starter ☐ “Homework” collection ☐ Other (please explain):

Student Response and Assessment Tools
☒ Multiple choice ☒ Multiple select ☒ True/False ☒ Yes/No
☐ Short open-ended response or fill-in the blank ☐ Longer open-ended response
If you are unable to provide a working sample of your questions, please list them below (8-10):
1. True or False? All waves are the same
2. Which of the following are examples of waves?
3. Which of the following waves has more energy?
4. When you look at a shirt the color you see is?
5. What do waves transfer?
6. Which of the following are the types of waves?
7. True or False? All waves transfer energy.
8. What kind of wave is sound?
Right/Wrong answers: Will there be right/wrong answers to these questions?
☒ Yes ☐ No
☐ Mixed (Some will have correct answers, other will not.)
Immediate corrective feedback: Will you pre-select correct answers to some of all of the questions and
display correct response to the class after the SRT activity?
☒ Yes
☐ No
Why or why not?
As I am using these questions as teaching points and none of them are free response, I feel it’s important to
have a right answer as a point for what the students need to know.
Describe what will happen AFTER the SRT activity?
After the activity with whatever time we have left I will show a real-world example of a wave and ask students
to describe the example based off the quiz and their prior knowledge.
How will the data be used?
This data will be used in several different ways and it will not be used for a grade. This data will serve as
information to myself so that I know where my students stand on prior knowledge going into the lesson as
well as giving that same information to each student. We as teacher seem to always wonder where students
stand before going into a new lesson, but it is also important for students to know where they stand heading
into a new unit. I also would use this data to gauge progress at the end of a lesson. This would be used as a
pre and post assessment to judge student progress over an entire unit.
