Dear Sir,

We Biocare Pharmaceutica authorize M/S Health Zone Distributor, Rawalpindi, to

quote, negotiate, supply and collect payments for our registered products with Drug Regulatory
Authority of Pakistan for PIMS Hospital for the Year 2020-21. As per following terms &
conditions:
1. You will collect orders from PIMS Hospital and convey it to Biocare
Pharmaceutica head office Lahore along with advance payment of the invoice.
2. You will be entitled 10% service charges of the invoice amount.
3. On clearance of payment you will again send the cheque of the institution to
Biocare Pharmaceutica.
4. Biocare Pharmaceutica will reimburse the payment to MS Health Zone
Rawalpindi in shape of cheque.
5. Income tax deduction will be responsibility of Biocare Pharmaceutica.
