1 COMPANY PROFILE/LEGAL STATUS

1.1 LEGAL STATUS 

1.1.1 Registration in Nigeria with CAC Contractor must submit all supporting documents that shows that the business is properly constituted  by law and 

1.1.2 Registration with DPR
1.1.3 Performance Record with Seplat
1.2
1.2.1 Company Organizational Structure
1.2.2 Local content adherence  Contractor must provide evidence of compliance with Nigerian local content act. This should include amongst other
registered in its country of origin. The following douments must be provided: 
1. Corporate Affairs Commission certificates (CAC 2 & CAC7) provided?
2. Last 3 Years Audited Accounts provided
Contractor must be registered in Nigeria with the Department of Petroleum Resources (DPR) to carry out works 
described in the ITT package
Does vendor have a valid DPR permit for the specifi work category?
Has the company previously  carried out work with Seplat? 
(For Seplat Repeat Vendors only) 
COMPANY PROFILE & ORGANIZATIONAL STRUCTURE
Contractor must provide all relevant company profile information which should include but not limited to the 
following:
1. Company's contact details; 
2. Description of Services Rendered.
3. Office address, Equipment Yard, 
4. Contact person(s),
5. Contact Details: Telephone (offfice, GSM), FAX, E‐mails  of company or company representatives,
6. Structured Company Organogram with logical reporting format
 
things:
1. Company's Local Content Act
2. Proof of Compliance to NCD Act

2 FINANCIAL/INSURANCE STATUS
2.1 Financial capacity to execute project 1. Provide audited account statement for the past two years; 
2. Provide tax clearance for the past two years.
3. Provide Bank Guarantee (if applicable)

3 TECHNICAL EXECUTION CAPACITY
3.1 Contact execution plan/work method  Provide detailed project execution plan to demonstrate how Contractor will
statement 1) execute the project within the proposed schedule
2) Provide plan to handle manpower and support logistics during execution. 
3) Provide detailed description of all work to be done 
4) Describe the methodology and strategy to be used on the projects.

3.2 Community affairs and site security  Contractor must provide a Community Affairs plan which will demonstrate how they intend to organize the Work 

proposals under the umbrella of SEPLAT's Agreements with Host Communities at Site. 
3.3 HSE management plan The Contractor must provide in writing his detailed their HSE policy document as we well as Project site specific HSE 
plan
• Note that the Contractor Safety plan must be in accordance with Seplat HSE contractor premobilization 
requirement                           
• Provides HSE policy signed by Management
• Provide Work Method Statement of the proposed task
• Provide a detailed JHA of how to manage the hazards associated with the project
3.4 Project schedule Bidder shall provide a Gantt chart showing how he intends to meet up with the timeline for the delivery of the Asset 
Verification and Asset register and Asset register  development/update program
3.5 Contractor's Personnel Experience Identify key personnel with proven understanding of Asset Information Management System requirements and can 
deliver Asset Management scope.  contractor should have experienced team including the following but not limited 
to;
    (a) Mechanical Engineer (Static and Rotating)
    (b) Instrument Engineer
    (c) Electrical Engineer
    (d) Process Engineer
Specific competencies should detail 
1. Previous  asset verification projects that personnel have been involved in.
2. Specific trainings and skillset obtained that are relevant to asset verification and asset register development . 
3. Specific trainings on specific software platforms used in Asset Information Management
3.6 Contractor's Capability  Vendor to show proof of Company's competence in Asset Verification. This should include inter alia the following:
1. Company Certification in any specific Asset Information Management software platform used in asset asset register 
development .
2. Technical proposal demonstrating company's capacity to carry out asset verification projects and list of satisfactory 
asset verification project executions. 
3. The CONTRACTOR shall also provide the following samples to provide evidence of understanding of the project 
scope:
a. Template for Data Gathering for PAV exercises
b. Template for Maintenance Matrix Development
c. Template for Criticality Assessment
d. Samples of Work Instructions/ Tasks operations for preventive maintenance 
e.Sample Reporting template for previous projects similar to the above
f. Sample of Redlining for PID documentation
3.7 Proof of Satisfactory execution of Asset  The contractor must the following:
Physical Asset Verification/Asset Register  1. A minimum of 3 LPOs  and Work completions  for similar Asset Verification and Asset  register development jobs 
build jobs  carried out for other companies                                             
2. Recommendation Letters from Multinationals for Satisfactory service Delivery
