NON INFECTED COUNTRIES)
FLIGHT ANNOUNCEMENT FOR INFLUENZA TYPE A / H1N1 SCREENING

Ladies and Gentlemen,

This is a health and safety announcement.
In the interest of your health and safety, we would appreciate your attention to
this Health Advisory, concerning Influenza Type A/ H1N1 by the Ministry of
Health, Malaysia.
If you have fever, or suffer from one or more of the following respiratory
symptoms, such as cough, sore throat, or shortness of breath, and have
visited *(Mexico, USA, Austria, Canada, China, Hong Kong SAR, Costa Rica,
Denmark, France, Germany, Ireland,
Israel, Netherlands, Italy, New Zealand, Republic Of Korea, Spain,
Switzerland, UK)* and other affected countries for the past two weeks, please
identify yourself to any of the crew members immediately.
Upon arrival, passengers from or traveling through any of the affected
countries mentioned earlier will be required to report to the Health Desk after
you disembark from this aircraft for health screening

Notes:

1. The above announcement must be made at the first port of call into Malaysia
and the last leg into KUL. Eg. ICN/BKI/KUL-announcement must be made into
BKI and into KUL.
2. (*)-Kindly check updates (via Dept circulars or at http://who.int/) on the infected
countries and announce accordingly.

FLIGHT ANNOUNCEMENT FOR INFLUENZA TYPE A / H1N1 SCREENING

Ladies and Gentlemen,

This is a health and safety announcement.
In the interest of your health and safety, we would appreciate your attention to
this Health Advisory, concerning Influenza Type A/ H1N1 by the Ministry of
Health, Malaysia.
If you have fever, or suffer from one or more of the following respiratory
symptoms, such as cough, sore throat, or shortness of breath, please identify
yourself to any of the crew members immediately.
Shortly, we will be distributing the Health Declaration Forms, and we request
that you fill in these forms truthfully.
Upon arrival, you will be required to undergo a Health Screening procedure
and the Health Declaration Forms will be collected by the Health Authority
personnel. Please extend your fullest cooperation.
We thank you for your attention and action on this Pandemic Influenza
Advisory.

(FOR ALL INTERNATIONAL FLIGHTS ARRIVING INTO MALAYSIAN PORTS THAT
HAS AT LEAST ONE REPORTED PASSENGER OR CREWMEMBER WITH FEVER)
FLIGHT ANNOUNCEMENT FOR INFLUENZA TYPE A / H1N1 SCREENING

Ladies and Gentlemen,

Upon arrival at the gate, the Malaysian Health Authorities will be boarding the
aircraft to assess the health of a passenger. Please cooperate by remaining in
your seats and keeping the aisles clear. The procedure will be carried out as
quickly as possible and disembarking will commence as soon as clearance is
given by the Health Officer.
Thank you for your cooperation.

Notes:

1. This announcement is to be made following the first Announcement A or B and if

there is any passenger or crewmember who has reported to have symptoms of
fever.
2. It is best that this announcement is made just prior to aircraft landing.
3. Please ensure that the Commander has been notified of the sick passenger and
has alerted ATC earlier.
