1. Automatic Transit Warranty of up to $500

Yes, I/We require a Transit Warranty of up to $500 on the cost price of all our consignments for a charge of $8.25
(exclusive of GST) per consignment.

2. Automatic Transit Warranty of up to $1,000

Yes, I/We require a Transit Warranty of up to $1,000 on the cost price of all our consignments for a charge of $11.90
(exclusive of GST) per consignment.

3. Optional Transit Warranty

No, I/We do not require Automatic Transit Warranty on all of our consignments. If I/We require Transit Warranty on
individual consignments I/We will declare the value or the consignment in the Transit Warranty section on the consignment
note. If no value is declared no Transit Warranty will be effected or charged.

I/We acknowledge that the Optional Warranty is charged at 2.15% of the value declared on the consignment note, with a
minimum declared value of $1,000. Consignments with a declared value under $1,000 will be charged at the minimum rate
of $21.50 plus GST.

Where the value of warranty required exceeds $10,000 I/We accept that a separate declaration form must be completed
and a quotation obtained. The warranty is limited to $10,000 unless StarTrack has confirmed in writing that the higher
warranty has been arranged.

I/We hereby accept that all goods consigned without Transit Warranty are done so at our own risk and I/We agree not to
hold StarTrack liable for any loss or damage.

I/We confirm that we have read and accepted the Transit Warranty Terms and Conditions set out on the reverse side of this page
which form part of the StarTrack Standard Terms and Conditions of Contract. I/We note that a copy of these terms and conditions
is available on the website www.startrack.com.au
