Question 1
a) Hazard is a potential source of harm to a worker. Basically, a hazard is the
potential for harm or an adverse effect for example, to people as health
effects, to organizations as property or equipment losses, or to the
environment.

b)
- Poor communication between the various design professionals
involved, e.g. engineers involved in conceptual design and those
involved in the supervision of execution of works.

- poor communication between the fabricators and erectors.

- Bad workmanship, which is often the result of failure to communicate


the design decisions to the persons, involved in executing them.

- Compromises in professional ethics and failure to appreciate the


responsibility of the profession to the community at large could also
result in catastrophic failures.

c) Practice for barriers and handrails is to minimize the risk of persons falling
from a stairway, balcony, landing, trafficable roofs or the like and for the
protection of openable windows in certain circumstances.

Question 2
a) The site may have to face power surges or power failures during operation
hours. Power surges may cause the electrical equipment used to be damaged
and become unusable whereas power failure during operation hours may
result in work failures or postpone which will slow down the constructions at
the construction site. At the same time, the workers at the construction also
may be vulnerable to accidents such as lift or elevator fall down and etc.
b)

 Work on new and existing energized (hot) electrical circuits is prohibited until all
power is shut off and grounds are attached.
 An effective Lock out/Tag out system is in place.
 Frayed, damaged or worn electrical cords or cables are promptly replaced.
 All extension cords have grounding prongs.
 Protect flexible cords and cables from damage.
 Use extension cord sets used with portable electric tools and appliances that are
the three-wire type and designed for hard or extra-hard service.
 All electrical tools and equipment are maintained in safe condition and checked
regularly for defects and taken out of service if a defect is found.
 Do not bypass any protective system or device designed to protect employees
from contact with electrical energy.
