A. GENERAL INSTRUCTIONS
1. This examination (booklet and answer sheet) contains 60 items cognitive questions.
Examinees shall manage to use the 2 hours allotted examination time.
2. Write your name and section on the upper left side of this sheet. No other words should be
written on these cells.
3. Do not use correction fluid or eraser of any kind to correct wrong name and section. Put a
horizontal line on the wrong name or section and write above or beside the corresponding
value.
4. Strictly CHEATING IN ANY FORM IS NOT ALLOWED otherwise punishable by
Philippine and school laws.

B. LETTER CHOICE INSTRUCTIONS

1. Use a No. 2 pencil or black inked ballpen only. Fully shade the circles like this .
2. Shade circle A, B, or C on the upper left side to indicate the set of test questions assigned to
you. Answers will be corrected using the key answer of the marked letter.
3. Shade one (1) circle for every number. WARNING. TWO OR MORE CIRCLES
SHADED OR MARKED, whether by pencil, black ballpen, fingernails, or any other
means, will be considered marked paper, thus your examination will be cancelled. DO
NOT LEAVE ITEMS UNANSWERED.
4. AVOID ERASURES. Answers with erasures which leave a mark will be invalidated. (If
you are not sure of your answer, leave the number blank, then go back to that number
before the end of the examination.
5. DO NOT FOLD OR MUTILATE. KEEP THIS ANSWER SHEET CLEAN.
Note to examinees: Papers/answer sheets will be checked using an Optical Mark
Reader (OMR). Test results are final when released, thus No Re-checking of Test
Papers.
