CONTRACT

KNOW ALL MEN BY THESE PRESENT:

The OSPITAL NG PALAWAN, with office address at 220 Malvar Street,

Puerto Princesa City, represented herein by AGNES B. PALAO ,RN, PhD,
Chief Nurse of Ospital ng Palawan and hereinafter as the PARTY OF THE
FIRST PART:

-and-

RENGEL HOTEL & RESTAURANT located at Valencia Street, Puerto

Princesa City, represented by Ms. MARILOU B. FUENTES, of legal age,
Filipino with the same address and hereinafter known as the PARTY OF
THE SECOND PART, being the General Manager.

WITNESSETH:

WHEREAS, the PARTY OF THE FIRST PART, is desirous of

engaging the services of the reputable establishment for the purpose of
providing the Venue, Lunch & 2 Snacks (A.M. & P.M.) for September 16-17,
2009 & Pack Lunch & Merienda (A.M. & P.M.) for September 18, 2009 to
the participants who will be attending Live-out Training for Basic I.V.
Therapy to be held on September 16-18, 2009.

WHEREAS, the PARTY OF THE SECOND PART is willing to

provide the Venue, Lunch, & Snacks for the above mentioned occasion
and purposes at reasonable cost.

NOW THEREFORE, in consideration of the foregoing premises and

of the terms and conditions hereunder stipulated, the parties agreed as
they hereby agrees, as follows:

1. That the PARTY OF THE SECOND PART shall provide Venue,

Meals and Snacks and other related services to the PARTY OF
THE FIRST PART for the period of 3 days, for 70 persons at
P360.00 net per person per day for September 16-17, 2009 &
P390.00 net per person per day for September 18, 2009.

2. That the payment of the amount and payable under this contract
shall be made upon consummation of the services provided by the
PARTY OF THE SECOND PART, subject to the provisions of this
contract and the usual accounting and auditing rules and
regulations.

3. That on the FIRST DAY OF THE ACTIVITY, the PARTY OF THE

FIRST PART shall pay in full amount as stated in the contract,
and in the event that the number of participants exceeds the
number stated in the contract, head count principle will prevail.
 4. That on the DAY OF THE ACTIVITY, head count principle will also
prevail for as long as it does not exceed the total number of
participants listed in the ONP Personnel Order.

IN WITNESS WHEREOF, the parties have hereunder set their

hands this 7th day of September, 2009 at Puerto Princesa City.

MD,MHA,CEO VI,FPOA,FPCS, Medical Center Chief I / OIC – Ospital ng
Palawan and hereinafter as the PARTY OF THE FIRST PART:

RENGEL HOTEL & RESTAURANT located at Valencia Street, Puerto

Princesa City, represented by Ms. MARILOU B. FUENTES, of legal age,
Filipino with the same address and hereinafter known as the PARTY OF
THE SECOND PART, being the General Manager.

WITNESSETH:

WHEREAS, the PARTY OF THE FIRST PART, is desirous of

engaging the services of the reputable establishment for the purpose of
providing 2 Snacks (A.M. & P.M.) to the participants who will be attending
the Refresher Course: Prevention of Nosocomial Intravascular Device
Related to Infection to be held on July 8-10, 2009 at Ospital ng Palawan –
Conference Room, Puerto Princesa City.

WHEREAS, the PARTY OF THE SECOND PART is willing to

provide Snacks for the above mentioned occasion and purposes at
reasonable cost.

NOW THEREFORE, in consideration of the foregoing premises and

of the terms and conditions hereunder stipulated, the parties agreed as
they hereby agrees, as follows:

1. That the PARTY OF THE SECOND PART shall provide Venue,

Meals and Snacks and other related services to the PARTY OF
THE FIRST PART for the period of 2 days, for 50 persons at
P150.00 net per person per day.

2. That the payment of the amount and payable under this contract
shall be made upon consummation of the services provided by the
PARTY OF THE SECOND PART, subject to the provisions of this
contract and the usual accounting and auditing rules and
regulations.

3. That on the FIRST DAY OF THE ACTIVITY, the PARTY OF THE

FIRST PART shall pay in full amount as stated in the contract,
and in the event that the number of participants exceeds the
number stated in the contract, head count principle will prevail.
