5. You should satisfy yourself that the plan serves your needs and that you can afford the premium throughout the term of the policy. If you need

6. Should you stop paying the regular premiums, the plan may continue for as long as there are sufficient units in the respective accounts. The
charges (insurance and service) for the respective accounts will be deducted through cancellation of units. If units within the accounts are
exhausted, these charges may be deducted through cancellation of units of certain selected accounts. Thereafter, depending on the
amount of units available in these selected accounts, benefits of the plan may lapse and cease to be in force at different points of time. This is
subject to your consent upon policy inception and is applicable for up to 1 year only. After 1 year we will obtain a fresh consent from you for any
subsequent cancellation of units for this purpose.

Premium Allocation
a) Table A shows the amount of unitised premium that will be allocated to buy units in this investment linked plan. For example, the
first year premium allocation is 40% out of RM1,000 premium, RM400 (less charges) will be used to purchase units.
b) The allocated premium is used to purchase units. A proportion of premium that is not allocated into units (i.e. premium paid less
allocated premium) is used to pay the commissions to agency force/other intermediaries.
c) The insurance charge is the amount deducted monthly from the value of units for providing insurance coverage. The insurance
charge is based on attained age next birthday, gender, smoking status, occupational class and the sum assured. It may be varied
from time to time. Other charges are charges other than the insurance charges, which include policy fees and other regular
charges.

1. All charges are subjected to change and may be varied from time to time by giving 3 months notice to policy owners.
2. Buying a regular premium investment-linked policy is a long-term commitment. It is not advisable to hold this policy for a short period of time
in view of the high initial costs.
3. In cases where the purchase involves a premium of sizeable amount, say RM5,000 and above, the prospect should consider purchasing
single premium investment-linked policy (rather than an annual premium policy) as single premium plans offer better allocation rates for
investment.

The X% and Y% returns above have been used respectively to represent the range of possible returns of PRUlink funds. The illustrated
returns are net after deduction of fund tax and the annual fund management charge.
It is emphasised that the X% and Y% assumptions are only for the purposes of illustration and do not represent the upper and lower limit of
the actual rate that may occur.

PRUlink investments
PRUlink Funds
PRUlink equity fund aims to maximise returns over medium to long term by investing in high quality shares listed on the Bursa Malaysia.
Unit Pricing
Unit price is calculated based on the market value of the underlying assets of the fund, divided by the number of units issued.
A PRUlink fund does not pay any dividend. All investment incomes and capital gains are automatically rolled up in its unit price.
Unit price is the single price at which units are created and cancelled.
Forward Pricing
Units are created and cancelled at the next pricing date following receipt of premium or notification of claim respectively.
Fund Manager
The PRUlink funds are internal funds managed by the investment team of Prudential Assurance Malaysia Berhad.
PRUlink assurance plan is a regular premium investment-linked whole life plan. Please refer to product's brochure for further details.
Options
. The payment of premiums may be stopped but the benefits above continue as long as there are units in the plan to pay insurance charge and
service charge.
. Units can be switched between PRUlink funds at any time.
. Top-ups can be made at any time. The minimum amount required for top-ups is RM1,000.
Charges
. Service charge of RM5 is levied monthly.
. Insurance charge based on attained age n.b., sex, smoking status, occupational class and the sum assured is also levied monthly.
. Fund switch fee is set at 1% subject to a maximum of RM50. One free switch is allowed every year.
. Top-up incurs a one-off service charge of RM25.
. Fees and charges above may be varied from time to time.
Benefits
. At maturity, PAP pays the value of units in Basic Unit Account, Protection Unit Account and Investment Unit Account.
. On surrender, PAP pays the value of units in Basic Unit Account, Protection Unit Account and Investment Unit Account.
Add-On Benefits
PRUmajor med 4 pays the benefits according to the plan selected below in the event of hospitalisation or out-patient treatment due to illness or
injury before age 70.
Co-insurance
* For Hospital & Surgical Benefits (except Hospital Daily Room & Board Benefit), you pay 10% of total eligible expenses subject to a Minimum Co-
insurance Amount of RM300 and a Maximum Co-insurance Amount of RM1000. The Company pays the balance. The Company reserves the
right to revise the Minimum and Maximum Co-insurance Amount once in every 12 months, by giving 30-day notice. For the Minimum Co-
insurance Amount only, the amount to be increased during such revision shall not exceed RM100.
** For Out-patient Treatment Benefits, you pay 10% of total eligible benefits and the Company pays the balance.
Notes
1. Overall annual limit and life-time limit do not apply to Emergency Medical Assistance.
2. Emergency Medical Assistance is a service provided by a contracted third provider and can be terminated by giving 90 days notice.
PRUmed pays the benefits below in the event of hospitalisation due to illness or injury before age 70. One unit of PRUmed pays
. RM50 per day (up to 100 days in a year) for hospital confinement.
. RM100 per day (up to 30 days in a year) while confined to an intensive care unit, and
. a lump sum up to RM2,500 (up to RM12,500 in a year) for a surgical procedure, depending on its severity rating.
PRUdisability provider pays the sum assured on each anniversary of claim for as long as the disability continues until age 65, on total & permanent
disability before age 60.
PRUpayor basic pays an annual benefit equal to BUA and PUA policy premium under the policy on diagnosis of a critical illness.
PRUprotector (PPT) is a regular premium rider for investment purpose only. It does not have any insurance cover. It serves as an additional buffer
within the Protection Unit Account to keep the overall account value from falling into deficit, especially during early durations of a policy.

The premiums and insurance charges of PRUlink products and optional benefits may be varied by the Company from time to time by giving 3 months
written notice.
Conditions Appendix
Death
If it is a suicide within the first year, all premiums paid will be returned without interest.
In the event of total and permanent disability before age 60, one lump sum payable under all policies with the Company on any one life is up to RM1
million plus bonuses (if any ). Any excess is paid in 9 equal annual instalments subject to proof of continued disability. Upon earlier death, the
balance of any unpaid instalments is paid immediately. The maximum payable under all policies on any one life is RM2 million plus bonuses (if any).
Total and permanent disability means the complete inability to engage in any occupation and to perform any work for remuneration or profit before
age 60. This also includes
(a) total and irrecoverable loss of sight of both eyes
(b) loss by severance of 2 limbs at or above wrist or ankle
(c) loss of sight of one eye and one limb
A total of 36 critical illnesses are covered under the Crisis Cover products
Heart attack, Coronary artery disease requiring surgery, Stroke, Cancer, Kidney failure, Paralysis, Major organ transplantation, Multiple sclerosis,
Fulminant viral hepatitis, Primary pulmonary arterial hypertension, Coma, Blindness, Heart valve replacement, Surgery to the aorta, Alzheimer's
disease, Deafness, Loss of speech, Major burns, Terminal illness, AIDS due to blood transfusion, Motor neurone disease, Parkinson's disease,
Chronic liver disease, Chronic lung disease, Aplastic anaemia, Muscular dystrophy, Poliomyelitis, Bacteria meningitis, Benign brain tumor,
Encephalitis, Full blown AIDS, Other serious coronary artery disease, Brain surgery, APALLIC syndrome, Major head trauma, Loss of limbs.
(a) symptoms of illness occur prior to or within 30 days after the policy has commenced, except for Cancer, surgery, Heart attack and Other
Coronary artery disease requring serious coronary artery disease in which 60 days waiting period applies.
(b) illness is directly or indirectly caused by the existence of AIDS or the presence of any HIV infection, except for "AIDS due to blood transfusion"
and "Full blown AIDS".
(c) the illness results directly or indirectly from congenital or inherited disorder and the age of entry is 6 months or less.
(d) the Life Assured is diagnosed to suffer from deafness and the age at entry is 2 years or less.
Conditions Appendix
Hospitalisation
Benefits are paid according to the limits in each benefit category of the medical riders in the event of hospitalisation due to illness or injury before age 70.
Cover for hospitalisation due to accidents commences immediately on acceptance but it only begins 30 days after acceptance for any other causes.
Medical benefits of PRUmed and Hospital Benefit are not paid as a consequence of :
(a) routine medical examinations or consultations.
(b) cosmetic or plastic surgery, dental care and treatment, organ and tissue donation, gender transformations or exploratory or experimental surgery
or any abortion performed due to procedures other than medical procedures necessitated by accidental injury, surgery or illness.
(d) tests or treatment related to infertility, contraception, sterilisation, birth defects, congenital illnesses, hereditary conditions, circumcision or any
abortion performed due to psychological or social reasons and consequence.
(e) treatment of mental illness and psychiatric disorders, self-inflicted injury while sane or insane, attempted suicide, abuse of alcohol and drug
addiction and abuse.
(f) any treatment or test in connection with AIDS , the presence of HIV infection and all sexually transmitted diseases.
(g) any injury or sickness arising from professional sports, scuba diving, racing of any kind, aerial flights (including bungee jumping, hang- hang-
gliding,ballooning, parachuting and sky-diving) other than as a crew member of or as a fare -paying passenger on a licensed passenger -
carrying commercial aircraft operating on a regular scheduled route or any hazardous activities or sports, unless agreed to by special
endorsement.
(h) war, invasion, act of foreign enemy, hostilities (whether war be declared or not), civil war, rebellion, revolution, insurrection, military or usurped
power, direct participation in riot, strike or civil commotion.
(j) any illness or condition (no matter when arising) in respect of which medical advice or treatment was received or recommended by a medical
practitioner within the 2 years preceding the issue date and which was not disclosed in the application for the contract.
The Company must be informed of any change of occupation, business or pursuit because it may affect the premiums, terms, conditions and benefits
of the products.
The Company reserves the right to increase the premium or to cancel the riders at any time by giving 90 days written notice.
Conditions
Hospitalisation
(b) Any injury or illness caused by war, invasion, act of foreign enemy, hostilities, (whether war be declared or not) civil war, rebellion, revolution,
insurrection, military or usurped power, direct participation in riot, strike or civil commotion.
(c) Any condition resulting from pregnancy, childbirth, miscarriage or abortion.
(d) Any form of dental care or surgery unless necessitated by accidental injury or illness involving oral surgery.
(e) Any cosmetic or plastic surgery (unless necessitated by injury or illness), any elective surgery, including donation of any body organ or sex
change.
(f) Hospitalisation primarily for investigatory purposes, diagnosis, X-ray examination, general physical or routine medical examinations; treatments
specifically for weight reduction or gain, convalescence, custodial or rest care.
(g) Any treatment or test in connection with AIDS or the presence of any Human Immuno-deficiency Virus infection or sexually transmitted diseases.
(h) Telecommunication charges, rental of television, mini-bar consumption or other medically unrelated services.
(k) Any injury or illness arising from professional sports, racing of any kind, scuba diving, aerial sports activities such as hang-gliding, ballooning, and
any other hazardous activities or sports unless agreed to by special endorsement.
(l) Any injury caused by violation or attempted violation of the law, or resistance to arrest.
(n) A pre-existing condition which shall mean the existence of any signs or symptoms for which treatment, medication, consultation or advice or
diagnosis has been sought or received during the two years prior to the commencement of the policy.
The Company must be informed of any change of occupation, business or pursuit because it may affect the premiums, terms, conditions and benefits
of the products.
The policy of PRUmajor med 4 (except for Emergency Medical Assistance) is non-cancellable but the Company reserves the right to revise the
premiums at any time by giving 3 months written notice.
Conditions
(a) A pre-existing condition where any medical condition in respect of which the life assured has been hospitalised during the 12-month period prior to
the inception date or any medical condition that has been diagnosed or treated by a medical practitioner including prescribed drugs within the 6-
month period prior to the inception date.
(b) Emergency Medical Evacuation or Repatriation or cost not approved in advance and in writing by International SOS (SOS) and/or not arranged
by SOS. This exclusion shall not apply to Emergency Medical Evacuation from remote or primitive areas which SOS cannot be contacted in
advance and delay might reasonably be expected to result in loss of life or extreme prejudice to the life assured's long term health prospect.
(c) Any event occurring when the life assured is within the territory of Malaysia (International Assistance) or within 100km from his place of residence
in Malaysia (Domestic Assistance).
(d) Any expense if the life assured is travelling outside Malaysia (International Assistance) or more than 100km from his place of residence in
Malaysia(Domestic Assistance) contrary to the advice of a medical practitioner or for the purpose of obtaining medical treatment or for rest and
recuperation following any prior accident or illness.
(e) Any expense if the life assured is not suffering from a serious medical condition or if the treatment can be reasonably delayed until he returns to
Malaysia (International Assistance) or the place of residence (Domestic Assistance).
(f) Any treatment or expense related to pregnancy, childbirth (except abnormal pregnancy or vital complication of pregnancy which endangers the
life of the mother and/or unborn children) and in any event of childbirth, miscarriage (spontaneous abortion) or pregnancy after the 6th month
(g) Any expense incurred for emotional, mental illness and psychiatric disorder as opposed to physical and strictly medical reason.
(h) Self-inflicted injury, suicide, alcohol abuse, drug addiction or abuse, sexually transmitted diseases, Acquired Immune Deficiency Syndrome (AIDS)
or any AIDS related conditions or diseases.
(i) Any treatment performed or ordered by a non-registered practitioner not in accordance with the standard medical practice as defined in the
country of treatment.
(j) Any event resulting from participation in war, riot or civil commotion or any illegal act including resulting in imprisonment or while serving in a
police or military unit.
(k) Any expense in respect of the life assured who is more than 70 years old at the date of the intervention unless otherwise agreed to in writing
by SOS.
(l) The cost of burial in the life assured's country of citizenship or Malaysia.
(m) Any expense related to accident or injury occurring while the life assured is engaged in mountaineering or rock climbing necessitating the use of
guides or ropes, pot holing, sky-diving, parachuting, ballooning, hang-gliding, deep sea diving utilising hard helmet with airhose attachments,
racing of any kind other than on foot, and all professional sports unless otherwise agreed to in writing by SOS.
(n) The cost of transporting the life assured by means of his owned or leased watercraft unless agreed to otherwise in writing by SOS.
