OSC Generic Oracle OLTP Applications Sizing Request Form

This form should be used for sizing of a third-party or a custom OLTP application using Oracle
database. Please contact osc_EBizDB_ww@oracle.com for any assistance during sizing. After
completion this should be emailed to osc_EBizDB_ww@oracle.com or faxed to the OSC at
+1.650.745.3545. The sizing will be sent to the local Oracle sales team who will contact you within 2-
3 working days.
