INTRODUCTION

The Fund for Public Health in New York, Inc. (FPHNY) is a 501c3 not-for-profit organization incorporated in
2002. FPHNY is the closely affiliated, non-profit partner to the New York City Department of Health and
Mental Hygiene (DOHMH), responsible for soliciting, administrating and receiving funds, and using such
funds to further assist the work of DOHMH. Through this partnership, DOHMH and FPHNY work to
advance the health and well-being of all New York City residents by supporting innovation, increasing
efficiency and leveraging DOHMH program capacity and reach.

The mission of the City of New York Department of Health and Mental Hygiene (“DOHMH”) is to protect
the health of all New Yorkers. Detailed information about DOHMH is available on its website, which can be
accessed through the main New York City website at www.nyc.gov.

The goal of the New York City DOHMH’s Healthy High Schools Initiative, located within the Bureau of
Chronic Disease Prevention and Control’s Physical Activity and Nutrition Program, is to facilitate changes
that improve the food environment inside and immediately surrounding high schools, thus promoting health
and wellness amongst youth. The initiative is focused on healthy and non-food fundraisers, sugar-
sweetened beverage reduction, and creative ways to raise awareness about nutrition.

STATEMENT OF PURPOSE

The FPHNY, on behalf of the NYC DOHMH, is requesting proposals from high schools to receive up to
sixteen available grants. Each school would be awarded $1,000 to design and conduct a campaign to
improve the food environment in their school. These mini-grants provide an opportunity for high school
students to explore and address the issue of what foods and beverages are available, promoted, and
consumed in and around their school building.

Campaign ideas must be linked to implementation of at least one component of the NYC Department of
Education (DOE)’s Wellness Policy. This policy can be found on the DOE’s website:
http://schools.nyc.gov/Offices/Health/GenProgServ/Wellness.htm.
The campaigns are to take place between September 2011 and January 2012. The grant funds will be
given to the awarded schools after the deliverables are accomplished. How the money is spent is at the
school’s discretion.
Note: only non-charter public high schools that have 9th through 12th grade are eligible for this
grant.

APPLICATION REQUIREMENTS

Awardees will be selected according to the completeness and content of the grant application. The
complete application will include the following: Section A, B, C templates are the last 3 pages of this
document.

SUMMARY SCOPE OF SERVICE AND PROJECT TIMELINE

Upon award of the grant, the FPHNY will enter into a formal contractual agreement with each school. This
contractual agreement will detail the scope of services, budget, and payment plan based on need and
mutual agreement.

The deadline for this proposal is May 18, 2011 by 5:00 PM. Proposals must be emailed by that time and
late proposals will not be accepted. The following schedule of events represents the best estimate of
the schedule that will be followed.

Late proposals will NOT be considered.

EVALUATION PROCESS

Evaluation Categories and Maximum Points Awarded


The proposal evaluation process is designed to award the contract to the vendor with the best combination
of attributes based upon the evaluation criteria. Preference will be given to plans that include robust
outreach strategies, maximum mobilization of youth, and the creation of visual components (e.g.
posters, photos, videos, etc.)
The maximum points that shall be awarded for each of the categories are detailed below.

1. Schools shall develop a campaign aimed at mobilizing the student body to change behavior and take action

This plan should specify the following and be organized under these headings: activities, timeline, reach, impact
evaluating, and sustainability. Proposed activities should show how youth will be mobilized around the selected
topic areas. Remember that current seniors will not be involved since this is for school year 2011-12. Preference
will be given to plans that include robust outreach strategies, maximum mobilization of youth, and the creation of
visual components (e.g. posters, photos, videos, etc.)
Work plan should not exceed one page, single or double-spaced, 10-12 size font.
