EDUCATION & CREDENTIALS - LCSW, LCADC, State of New Jersey - MSW, Rutgers University, New Brunswick, NJ - BA Allegheny College, Meadville, PA EXPERIENCE Acute Partial Hospitalization/MICA, Bergen Regional Medical Center, Paramus, NJ, October, 2007 - present, Social Worker - Assist with planning and implementing a comprehensive social services program that provides for counseling and other support services for 60 patients. Partic ipate in daily round case reviews - Identify medical-related social needs of seven patient caseload. Provide appr opriate services to meet the individual, as well as collective needs of patients , in short term treatment setting. - Perform initial psychosocial evaluation/assessment of each patient to be plac ed in the medical record. Active cases require ongoing written documentation - Coordinate interdisciplinary care planning at time of admission for each pati ent - Encourage and provides alcohol and drug dependency treatment depending on nee d. (80% of patient population MICA identified) - Evaluate each patient in caseload, mental and social needs then formulates a treatment plan. With patient participation, identify specific problems, goals an d approaches. Periodically re-evaluate with the patient and make adjustments acc ordingly - Facilitate two to three 60-minute group therapy sessions per day, 8 to 16 pat ients per group. Group sessions are divided into three categories: psychotherapy , education, and coping skills - Develop and implement new curriculum for activity groups using evidence based interventions - Plan and implements family meetings with patient and treatment team when appr opriate. Extensive knowledge of family therapy - Schedule individual counseling sessions. Work directly with patients to promo te the preservation of the patient's mental health - Coordinate discharge planning - Consult with the psychiatrist and other administrators regarding interdiscipl inary issues - Maintain a written record of the frequency and nature of all patient and staf f consultations, weekly updates on caseload and daily notes on each patient atte nding group session. These notes meet or exceed JACHO, Medicaid, and Medicare re quirements (approximately 35 notes per day) PROJECT COPE, Montclair State University, Montclair, NJ, September 2006 - April 2007 Intern Prevention Counselor - Offered violence and drug prevention counseling for Paterson high-risk youth, working in concert with school guidance counselors and principles - Joined program at initial stage and, with team, developed intervention strateg ies - Trained in Bowen family system theory through lectures, discussion groups and case study sessions.
