I am a Nurse with something special. Along with my nursing education I
come with a pocket full of proven skills such as assessment, planning,
critical thinking, analysis, teaching, communication, negotiation, and
supervision. Through my experience as a long term acute care floor
Nurse, school Nurse, IAT (home care) Nurse, ALH Nurse, volunteer Nurse
and a CNA my I have grown. I like to use my head, my hands and my
heart. I like a challenge; I like to get things done. I hate to be
told it can't be done, I love to learn. I love a busy, fluid day. This
is when I KNOW I make a difference. I look forward to growing my
skills as a nurse using my unique talent, personality, perspective and

2009 - 2008 Ready Care/Job Ready: (Home Care Medicaid Waiver Service)
IAT Nurse
IAT Nurse for CCMC & MRDD clients responsible for quarterly client
visits, developing training checklist and IAT (training care plan) for
in home staff (developed new IAT format) Training staff (care of
critically ill children and adults) in concert with family. Providing
in home training client specific manual. Preparing annual Nursing
Level of Care (NFLOC) for approval by the State to maintain Medicaid
Waiver services. (as part of the wavier Plan of Care (POC)
On-call/back-up ALH Nurse for assisted living home (ALH) of serve
disabled clients. training and supervision of Unlicensed Personal
(ULP) for total care pt including ADL's, trach care ,g tube feeding
and seizure precautions,ect. Attend rounds weekly at local hospitals
to review NICU and PICU admissions and their needs for screening and
tracking for waiver services. CCMC screening. Care Coordinator for the
Medicaid Waiver programs including: Older Alaskans (OA), Older Persons
with Disabilities (OPD), and Mentally Retarded/Developmental Delayed
(MRDD). Includes monthly visits, documentation, billing, and
amendments as service requirements demand, annual renewal of POC. TB
clinic. Certified CPR trainer and instructor trainer. Other duties as
