Cues:
Subjective: “ok raman maskin walay tabon amo galloon, wa man mi nangasakit kaluoy sa Ginoo”
Objective:
 uncovered water container
 the container is brownish with dark spots and uneven yellowish color of water
Family Nursing Problem : Inability to recognize the possible complication of the
problem identified due to uncovered water container

Goal of Care: After 4 hours of nursing of nursing interventions, the family will maintain to
have a clean container with a cover and will be able to modify a cover on it, if resources are
available

Objectives: After 1 hour of nursing intervention, the importance of having a clean, covered
container will be realized and understood by family members through health teachings

Interventions:
1. assess the family’s level of awareness about the importance of having a clean
water container with an appropriate cover
- to identify their capacities to recognize the possible diseases they can get
from uncovered water container

2. look for a quiet place where the client can give their full attention listening to
health teachings
- to easily absorb the health teachings given

3. advise the family to constantly clean their water container

- to prevent from acquiring diseases because of the microorganisms which may
lodge on the container

4. encourage mother to constantly apply the measures taught to them by the

Student Nurse such cleaning the container and changing the water as often as
possible
- to protect themselves from instances which can possibly cause an illness

5. emphasize health threats it may bring to them such as diseases like diarrhea
- to develop self-responsibility to protect themselves from different illness

6. discuss to the client the disadvantages of having their container uncovered

- to enhance their usual practices

7. provide suggestions in solving existing problem such as making use of plastic

cover or any clean equipment that covers the entire mouth of the gallon/pitcher
- to supplement information which might be helpful to the enhancement
of their usual practice

8. provide clear picture on their mind on what are the possible effects and problem
they may encounter on such practices

9. emphasize to clean the water container using soap and water even thrice a week
- to maintain cleanliness

Evaluation: Goal Met. After 4 hours of family home visit with nursing interventions,
the family was able to clean the water container and was able to put a cover on it through
modifying some clean items from their house.
