Selected Contributions & Successes
* Challenge - Pulsating (ultra-low) pressures were
destroying pressure instruments in an automated loop.
Action & results: Inserted controlled restriction in the
impulse (i.e. sensing) line and extended the length
of this line, reducing the amplitude of the pulses by a
factor of approximately 10 and stopping the destruction
of instruments.
* Challenge - Catastrophic failures (plug disengagement)
of 10" throttling valve occurring every 5 years.
Action & results: Performed Root Cause Failure Analysis
(RCFA) to diagnose and determine that out-gassing of CO2
(from specialty liquid) within valve at its Vena Contracta
and subsequent pressure recovery caused "buffeting" or
jerking of valve stem. Metal fatigue caused stem/plug
connecting-weld to fail, dropping plug to bottom of
valve housing. Specified and installed new "low recovery"
valve which keeps pressure higher, preventing CO2 from
out-gassing and stopped the instability.
* Challenge - New floating roofs and Geodesic domes added
to several refinery storage tanks, requiring addition
of costly underground wiring for new hi-hi level switch
system to each tank for remote alarming to avoid
overfilling and damage to the new domes.
Action & results: Recommended wireless transmission from
each tank's switch to a remote radio receiver near control
central (DCS/TDC 3000 location) and to use aboveground
wiring for the short distance from receiver to DCS system,
resulted in cost savings of $200,000 and tested successfully.
